Scientists Think Protein in Knee Joint May Ease Arthritis Knee Pain

In healthy knees, synovial fluid is the lubricant that allows the joint to glide.  In an unhealthy arthritic knee there are decreased amounts of synovial fluid.  In the past few years pharmaceutical companies have come out with rooster comb injections (aka gel injections, hyalgan injections, hyaluronic acid injections) to try and replicate the effects of synovial fluid.  However, research indicates that the active protein or molecule in those injections only lasts for a few days in the knee joint before it is absorbed into the bloodstream. 

That is why Saleem Abubacker, a 23 year old PHD student at the University of Calgary in Canada and his colleagues have been researching a different molecule then the one found in current knee injections.  Abubacker and his team are studying the effects of lubricin, a protein found in synovial fluid and on the surface of the knee joint.  They are injecting lubricin in unused cow joints from butcher shops and putting them in a machine that pushes them and twists them like human knee joints.  Preliminary studies suggest that lubricin lasts for longer periods of time in the knee joint (28 days) compared to the current molecule used for injections on the market, and it has shown to slow down the breakdown of cartilage.  They are also finding that the molecule forms building blocks, so as it grows it makes a difference in lubricating the knee.  Abubacker thinks that some patients with arthritis knee pain may have more of the single molecule then the long chain and that by restoring that balance of lubricin they may have less arthritis knee pain.

Obviously, more advanced studies need to be completed with lubricin, especially on human knee joints, but who knows this may be the next type of knee injection in the future.

A Meniscus Tear…

The meniscus is the rubbery cartilage like material that sits between the shin bone and the thigh bone.  There are two menisci one on the inner part of the knee (medial meniscus) and the other is on the outer part of the knee (or the lateral meniscus). Over the years, there have been numerous studies on the effects of cold laser therapy for arthritis knee pain.  Recently Dr. Alexandra K. Schnee, D.C. and Dr. Robert Hanson, D.C. published the results of their clinical findings in an abstract that was submitted, peer-reviewed and accepted by the North American Association for Laser Therapy. The abstract entitled  Low-Level Laser Therapy And Its Effects On The Quality of Life for Patients Suffering From Osteoarthritis Knee Pain in the Medial Compartment:  A Detailed Analysis By Questionnaire will be presented at NAALT’s annual conference in September. 

In summary, Dr. Schnee treated 119 knees, representing 86 patients over a period of 10 months.  The only patients who participated in this study were those diagnosed with either moderate to severe arthritis on the medial aspect of the knee (inside part-where the knees touch).  Patient’s filled out detailed questionnaires on their final treatment and 79.8% of them reported that their knees were moderately to a great deal better.  It was concluded that low-level laser therapy or cold laser therapy for arthritis knee pain helps improve the overall quality of life for patients suffering with chronic knee pain.  The following is an excerpt from the abstract.

LOW-LEVEL LASER THERAPY AND ITS EFFECTS ON THE QUALITY OF LIFE FOR PATIENTS SUFFERING FROM OSTEOARTHRITIS KNEE PAIN IN THE MEDIAL COMPARTMENT: A DETAILED ANALYSIS BY QUESTIONNAIRE

Introduction:  The aim of this study was to assess the effects of low-level  laser therapy (cold laser therapy) for patients specifically diagnosed with moderate to severe (bone on bone) osteoarthritis knee pain in the medial compartment of the knee by using questionnaires. 

Methods:  Patients were treated at the DFW Spine & Joint Clinic from June 2010 thru April 2011.  There were a total of 119 knees treated representing 86 patients.  One hundred nineteen questionnaires were completed and collected after the patients treatment.

Results: There were a total of 86 patients diagnosed and treated for moderate to severe (bone on bone) osteoarthritis in the medial joint compartment with symptoms of knee pain.  This diagnosis was a result of either an x-ray or an MRI. Out of those patients 33 had both knees treated and 53 patients had only one knee treated.  There were a total of 34 males and a total of 52 females, with a mean age of 66.13 years.

 Patients were asked to rate their overall change in activity, limitations, symptoms, emotions, and quality of life after completing 20 treatments using a detailed questionnaire.  Patients who had both knees treated were asked to fill out two different questionnaires, one for each knee treated.   Overall 79.8% rated the overall improvement of their knee either moderately better to a great deal better.  None of the patients rated that the treatments had negative effects on their quality of life or that they experienced more knee pain on the completion of their 20th treatment.

Conclusion: Our results indicate that Low-level laser therapy (or cold laser therapy) significantly improves the overall quality of life and related factors for patients diagnosed with moderate to severe (bone on bone) osteoarthritis knee pain in the medial compartment of the knee making this therapy an ideal treatment for patients with chronic knee pain.

Inflammation: 

According to the periodical, Lasers in Surgery in Medicine, cold laser therapy increases the production of nitric oxide which helps reduce inflammation, improves oxygenation to the area and improves the overall healing ability of the injured tissue…It is not uncommon for patients with knee pain to have inflammation or swelling in the knee.  When soft tissue (muscles, ligaments, and tendons) becomes injured, inflammation is common as a result of the body responding to protect the injured area.  Pain is a byproduct of inflammation caused by the extra fluid in and around the joint exerting pressure on the sensitive nerve endings.  Naturally our bodies produce nitric oxide to reduce swelling in an area, which acts to relax muscles, dilate capillaries, improve blood flow and help protect against further cell injury.  Cold laser therapy increases the production of naturally occurring nitric oxide, which reduces inflammation, improves oxygenation to the area and improves the healing ability of the injured tissue.

A report published online on July 7th, 2011 in Arthritis Care & Research by researchers at the University of Southampton in the United Kingdom found that women who gained weight over a fifteen year period had greater knee pain in both knees.  The study followed 594 females body mass index over a period of fifteen years and discovered that an increase in body mass index was associated with greater knee pain in both knees regardless of the severity of osteoarthritis diagnosed on the x-rays. 

Basically, this study reported on something women already intuitively know… that is the greater the weight gain the more stress it places on the knees.

Weight Loss Surgery May Help Reduce Knee Pain, But it Doesn’t Eliminate Years of Joint Damage

The biggest misconception amongst patients who are obese is thinking that the arthritis in their knee will go away after bariatric weight loss surgery. Even though the surgery will help improve their overall health and reduce knee pain it will not improve the arthritis in the knee joint. This is according to researchers and a new study presented at the Annual Meeting of the American Academy of Orthopaedic Surgeons.

The study followed 10 patients that lost an average of 51 pounds after weight loss surgery.

All of them reported a reduction of knee pain and increased function a year after the surgery. Moreover, all of them had irreversible damage to the knee joint which didn’t disappear with weight loss.
