My knee pain was awful; I don’t even want to remember it.  I couldn’t even walk at times.

I started using a cane when we went to the store, or anywhere, we (often) had to go back home, because my knee (would start) hurting.  I (just) couldn’t walk anymore from the pain. I would be limping.   Also the pain kept me awake at night and I couldn’t sleep.

Verla Y., from Benbrook Texas, is an 80 year old female that came into my office complaining of severe pain in her left knee. Most of the pain she felt was under her knee cap and on the inside of her knee (where the knees touch). She could not stand  or walk for more than a few minutes at a time without knee pain.  She also felt some intermittent pain in the back of her left knee which limited her ability to extend and bend her knee.

She had been to her primary care physician in the past but he never took x-rays or recommended surgery.  He just prescribed pain medication which gave her temporary relief.

Her knee pain had gotten progressively worse over the past six years but it became more severe and limiting in the past few months.  When she initially came into my office she couldn’t walk, stand, vacuum, climb stairs, lift or carry items that weighed more than a few pounds without her knee hurting.

Initially, I examined and took x-rays of Mrs. Verla’s left knee.  Upon examining her knee she had visible swelling all around her knee joint.  She also felt pain to the touch on the medial aspect of her knee and below her knee cap.  The x-rays showed signs of severe arthritis in both the medial compartment (the inside of the knee where the knees touch), and behind her knee cap.  Her x-rays also showed ‘bone on bone’ on a small part of the medial aspect of the knee. Mrs. Verla knew that she had waited a long time to address her knee problem, so by the time she came to see me it had already severely deteriorated.

After reviewing and explaining her x-rays to her we discussed all of her options.  I also explained cold laser therapy, exercises, and the other modalities we would be using during treatments.  I was very clear that because of the stage her knee was in there was no way we could get her knee feeling 100% perfect again.  She also understood that if she chose not to do any type of care her knee would progressively get worse and she would eventually need a knee replacement.

Since she did not want her knee to get any worse and she did not want surgery she decided to start treatments.

After a few treatments, Mrs. Verla started to respond favorably to care (just like I thought she would), and after several weeks she really noticed a significant reduction in the swelling and pain in her knee.

Only 48% Were Able to Return to Their Normal Activities, Several years After Arthroscopic Surgery

A long-term study to determine the outcomes of patients who underwent arthroscopic  partial  lateral meniscectomy (partial removal of the lateral meniscus) for lateral meniscus tears was conducted in otherwise normal knees.  The study reviewed patient’s activity level, clinical, and x-ray results.

Patient’s who underwent the surgery were followed up with a minimum of 5-8 years later.  They were evaluated by questionnaire, x-rays, and knee examinations.  Initially, only 85% of those who underwent surgery were able to temporarily return to pre-injury activity level.  However, this percentage dropped to 48% at the time of follow-up.     The study concluded that the activity level of patients undergoing arthroscopic surgery for partial lateral meniscus removal is likely to deteriorate with time.  The study also concluded that patients should be counseled concerning long-term expectations regarding their knee pain.

Is Arthroscopic Knee Surgery a ‘cure all’? Not if 32 out of 53 patients had to repeat the procedure…

Yes, arthroscopic surgery does help some patients get relief from knee pain but on the other hand so do shots, and medications.   At least with shots and medications a doctor is not removing cushion from the knee joint as in the case of arthroscopic knee surgery. (Unfortunately, there are other side-effects that arise from getting shots and taking medications that I will discuss in another blog.)  After years of published research, we now know that IF arthroscopic surgery does help relieve pain, by removing the meniscus either partially or in its entirety,  the effects are often temporary in nature (a few months or years) and in the long run the surgery leaves the knee vulnerable to future tears and arthritic changes.

In a retrospective review of the results of  arthroscopic procedures that were performed for the treatment of an acute or a chronic meniscal lesion, with or without an associated ligamentous lesion, eighty meniscal tears were identified (in seventy-five patients) that had been assumed to be stable. Of the seventy-five patients, fifty-two had been followed for two to ten years. A repeat arthroscopy was performed on thirty-two patients at an average of twenty-six months after the original arthroscopy. This means that over 61% of patients who underwent arthroscopic knee surgery had to repeat it again because it was unsuccessful the first time.

Severe Osteoarthritic Changes Occur in Knees Over Forty, after Arthroscopic Knee Surgery

I’ve written previous blogs about the subject of arthroscopic surgery, and in most cases I believe it is a useless procedure  because  in the long run it creates more osteoarthritic changes in the knee  which ultimately leaves the patient with the possibility of having to get a  total knee replacement later in life.  Multiple studies support this idea and clearly indicate that osteoarthritic changes become more prominent after arthroscopic knee surgery.  A recent study conducted at the Department of Traumatology in Austria also supports these studies.   In this current study, arthroscopic knee surgery was performed on 284 patients to remove part of a torn meniscus.

After the surgery was completed x-rays of the knees were taken.  Patients were then X-Rayed and evaluated again 53.5 months after the surgery.  Two hundred forty-seven patients had been treated for medial meniscus tears and 37 for lateral menisci tears. Preoperative radiographs were compared with those at follow up and were classified. The results were analyzed statistically.

Osteoarthritic changes were classified as being worse in 38% of the patients after medial and in 24% of the patients after lateral arthroscopic partial meniscectomy (removing part of the meniscus).   Patients who were older than 40 years of age and who had undergone arthroscopic partial medial meniscus removal had a significantly higher rate of osteoarthritis than patients who were younger than 40 years of age. The study concluded that partial medial or lateral meniscus removal leads to a significant increase of osteoarthritic changes, after arthroscopic surgery especially if you are over 40 years of age.

Before doing Dr. Schnee’s Cold Laser Knee Program I could not walk very much or for a long period of time.  I could not get up and out of a chair, off of the bed, or the commode without a lot of knee pain in both knees.  The up and down movement on my knees was very difficult.  I was told by two doctors that I needed knee replacements but I didn’t want surgery  so  instead  I had shots in both knees and I took pain medications to help relieve the knee pain.  The shots and pain medications didn’t really help me.  That is when searched for other options and I started Dr. Schnee’s Cold Laser Knee Program.

Since starting Dr. Schnee’s Cold Laser Knee Program I can now sleep much better and without a pillow under my legs.  I stand and walk for longer periods of time without pain.  Stairs are easier to climb up and down and getting up and down is easier.  Honestly, everything is now easier without the pain in my knees.  I am very happy with results of the treatments.  One knee isn’t hurting at all now and the other knee only hurts when I overdo it.
