Why Hasn’t My Doctor Told Me About Cold Laser Therapy?

That’s a good question.  I don’t know why YOUR knee doctor hasn’t shared the benefits of cold laser therapy with you, but I think I know the answer…  First, let me just state that our office gets daily referrals from orthopedists, internal medicine doctors, rheumatologists, pain management doctors and primary care physicians.

Most physicians understand that not all patients want, need or respond well to surgery, injections or pain medications.  Physicians refer patients to our office to undergo our Cold Laser Knee Program because it is considered safe, and effective, and it’s definitely worth trying if other knee treatments have failed, and before any further knee surgery is recommended or performed.

Now let me get back to answering the question.

There are two obvious reasons why your doctor hasn’t told you about cold laser treatments.

The first and most obvious reason is that they are probably unaware of the technology.   Even though there are an increasing number of doctors using cold lasers, and are becoming more educated about the benefits of the treatment there are still many more physicians that do not know about it.

The other reason your doctor may not have told you about the treatment is because they are narrow minded, and they just don’t want to change their method of treatment. These doctors are skeptical about everything except for the techniques they use.  It doesn’t matter what overwhelming evidence you provide them they still consider everything other than ‘conventional medicine’ a sham.  (Ironically, ‘conventional medicine’, for example, prescription medication and surgery was ranked one of the leading causes of death by the CDC.   In my opinion, ‘narrow minded’ physicians are one of the reasons why the U.S. is only ranked 37th in health care by the World Health Organization.)  I wonder sometimes if this group of physicians is truly acting on behalf of the patient or only looking out for themselves.  After all, if your knee pain significantly improves or is eliminated after completing our Cold Laser Knee Program then why would you go back to the surgeon?

The bottom line is…if you’ve tried medications, knee injections and/or knee surgery and those options haven’t helped you than don’t wait for your knee to get worse.  Your quality of life doesn’t have to suffer just because you orthopedic doctor hasn’t referred you for or told you about cold laser treatments.

Laser Education 101: The History of Lasers

LASERs were first theorized by Albert Einstein in 1917.  In 1960, Dr. Endre Mester, a Hungarian scientist, invented laser technology.  Prior to being FDA Cleared in the U.S. cold lasers were being used in Europe, Japan and China to treat acute and chronic conditions. Do The Series of ‘Gel Like’ Injections for Knee Pain Really Work?

I’m referring to Hyaluronic acid injections aka rooster comb, gel  injections, viscosupplements, SYNVISC®, Hyalgen®, Euflexxa®, Supartz®, Orthovisc®, etc.

In my opinion the answer to this question is NO, NOT REALLY. Ultimately though you have to decide for yourself… Let me explain…

I have personally treated and talked to many patients who have had the series of three to five injections listed above (3-5 injections are usually administered over the course of a few weeks. Recently, SYNVISC® came out with one injection).  Most patients noticed little to no knee pain relief after the injections.  Some noticed that their knee pain and swelling got worse after the injections and they developed what’s known as a ‘hot knee’.  Others developed an allergic reaction to the shot and were unable to complete the series. Of the few that responded favorably to the injection theyonly  noticed temporary pain relief (which in all fairness the injections do claim that the relief is only temporary and usually lasts for up to 6 months).

There are multiple studies indicating that Hyaluronic injections are as effective as placebo injections,[1] [2] [3]and that the effect of anti-inflammatory drugs is 48 times greater than the effect of Hyaluronic acid injections (or rooster comb injections).[4]

One study indicated that the immediate pain relief patients experienced was not due to the injection itself, but from draining the fluid in the knee, a step doctors take prior to administering the injection to prep the knee.

Most patients mistakenly think that the knee injections add ‘cushioning’ or cartilage to their knee.  Unfortunately, this is not the case! IF the injections are successful the theory behind them is that they help lubricate the knee joint. The injections do NOT stimulate cartilage or increase the amount of cartilage in the knee.

Based on my personal experience and the multiple studies I have read (not the ones funded by the pharmaceutical companies, but ones written by a third party) about the injections.  I believe that there isn’t any real evidence that proves their effectiveness in the treatment of osteoarthritis knee pain.

On the other hand, I do believe that an individual should leave knee surgery as their very last option, so it’s certainly worth giving gel injections a ‘shot’.

Are Synvisc Shots (Rooster Comb) Better then Cortisone Shots for Osteoarthritis Knee Pain?

The Journal of Bone and Joint Surgery (American) published a study comparing the results of cortisone and Synvisc injections in patients diagnosed with osteoarthritis knee pain. One hundred patients with knee osteoarthritis were randomized to receive an injection in the knee joint of either Hylan G-F 20 (Synvisc) or a corticosteroid, and they were followed for six months. The patients treated with Hylan G-F 20 (Synvisc) received one course of three weekly injections. The patients treated with the corticosteroid received one injection at the time of enrollment in the study, and they could request one more injection any time during the study. An independent, blinded evaluator assessed the patients with the Osteoarthritis Western Ontario and McMaster UniversityIndex (WOMAC), a modification of the Knee Society rating system, and the visual analog pain scale.

Researchers concluded that after a six month follow up that there were no detected differences in knee pain or function between patients treated with Hylan G-F 20 injections (Synvisc) then those treated with a cortisone shot.  Interestingly, women demonstrated significantly less response to knee treatments than men did for both treatments on all three outcome scales.

Omega-3 Fatty Acids Help Boost the Effectiveness of Glucosamine

Recently in the Journal of Advances in Therapy German researchers discovered that adding omega-3 fatty acids (from fish oil) to glucosamine sulfate improves the symptoms of arthritis compared to just taking glucosamine alone.

One hundred seventy seven men and women diagnosed with moderate to severe hip or knee osteoarthritis were randomly placed in two groups.  The first group received glucosamine sulfate without omega-3 and the other group received glucosamine sulfate with the omega-3 fatty acid. Both groups took the supplements for 26 weeks.

Eighty percent of the group noticed a reduction in pain, 44% of those were from the group that received the combination of glucosamine and omega-3, and 32% of those were from the group that received only the glucosamine sulfate.

The authors of the study remarked that glucosamine sulfate improves cartilage metabolism, and omega-3’s help further reduce degradation of cartilage by reducing inflammation which ultimately helps reduce pain and swelling.

Note:  Glucosamine/Chondroitan/MSM should be used as a SUPPLEMENT to care. It is not a ‘cure all’ but if the correct dose and correct type of glucosamine is taken daily then you will notice a difference overall symptomatically.  (Please keep in mind that not all glucosamine is created equal.  This is a topic that I discuss with my patients because there seems to be a lot of confusion about it.)

Knee Pain After Arthroscopic Knee Surgery Reduces Activity Level for Patients

A long-term study to determine the outcomes of patients who underwent arthroscopic  partial  lateral meniscectomy (partial removal of the lateral meniscus) for lateral meniscus tears was conducted in otherwise normal knees.  The study reviewed patient’s activity level, clinical, and x-ray results.

Patient’s who underwent the surgery were followed up with a minimum of 5-8 years later.  They were evaluated by questionnaire, x-rays, and knee examinations.  Initially, only 85% of those who underwent knee surgery were able to temporarily return to pre-injury activity level.  However, this percentage dropped to 48% at the time of follow-up.     The study concluded that the activity level of patients undergoing arthroscopic surgery for partial lateral meniscus removal is likely to deteriorate with time.  The study also concluded that patients should be counseled concerning long-term expectations regarding their knee pain.

Study Proves If Your Over Forty Your Knee Will Be Worse Off After Undergoing Arthroscopic Knee Surgery

Forty-nine patients who were more than forty years old when medial meniscectomy (arthroscopic surgery to remove the medial meniscus due to a tear) was performed returned for follow-up evaluation one to twelve years postoperatively. Pain was present in 62.2 % of patients who underwent knee surgery to ‘clean out’ degenerative tears (tears due to arthritis), and in 75% of those who underwent surgery to ‘clean out’ traumatic tears (tears due to a knee injury).

The medial (inner) joint space was significantly thinner in patients who had knee surgery to clean out degenerative tears versus those with traumatic tears.

 

The x-ray findings of osteoarthritis were significantly more severe on the side operated on.

The angle of the knee was relatively more ‘bow legged’ on the surgically treated knee (mean, 5.3 degrees) versus the knee that did not undergo knee surgery.

In conclusion, before committing to undergoing knee surgery make sure you try all conservative knee treatment options first! Cold Laser Therapy for Knee Osteoarthritis Is an Effective and Side-Effect Free Therapy in NSAID Resistant Knee Pain

In order to ease osteoarthritis knee pain many knee pain sufferers will take NSAIDs (non-steroidal anti-inflammatory drugs). NSAIDs can either be prescribed or be purchased over-the counter.  The most popular drugs are Mobic, Naprosyn, Ibuprofen (Advil and Motrin), and Alleve.  Unfortunately, many of these drugs have long term side-effects and are not EFFECTIVE OR SAFE for those suffering with osteoarthritis knee pain.  Many knee pain sufferers have consumed NSAIDs for so long that they are no longer a viable treatment, and they become resistant to the drugs.

Previous clinical studies have shown effectiveness of low-level laser therapy (cold laser therapy) in reducing knee pain and inflammation. In a recent study, after 6 weeks of NSAID (non steroidal anti-inflammatory drugs) consumption, 386 unsatisfied patients were treated with cold laser therapy.   Cold laser therapy was conducted to determine if cold laser therapy was an effective knee treatment for osteoarthritis knee pain.   After completing just 12 sessions the effects of cold laser therapy were assessed.   Researchers discovered that there was a statistically significant improvement in pain, night pain, morning stiffness, walking pain, inflammation, and range of motion after the cold laser therapy treatment was performed.

The study concluded that cold laser therapy is an effective and side effect-free adjunctive therapeutic modality for knee pain osteoarthritis even if the patient is NSAID resistant.

Years ago, shots of various mixtures did not help my joint arthritis knee pain, & I considered Radio-Frequency treatment. However, studies revealed that hot-radiation was counter-productive, harmful to body tissues, nerves & cells. When directional-lasers, low-power/controllable were developed, I was excited about their use in the body. Yet no medical practice/use was available.
