Cold Laser Therapy for Arthritis Knee Pain

Cold Laser Therapy for Arthritis Knee Pain

Over the years, there have been numerous studies on the effects of cold laser therapy for arthritis knee pain.  Recently Dr. Alexandra K. Schnee, D.C. and Dr. Robert Hanson, D.C. published the results of their clinical findings in an abstract that was submitted, peer-reviewed and accepted by the North American Association for Laser Therapy. The abstract entitled  Low-Level Laser Therapy And Its Effects On The Quality of Life for Patients Suffering From Osteoarthritis Knee Pain in the Medial Compartment:  A Detailed Analysis By Questionnaire will be presented at NAALT’s annual conference in September. 

In summary, Dr. Schnee treated 119 knees, representing 86 patients over a period of 10 months.  The only patients who participated in this study were those diagnosed with either moderate to severe arthritis on the medial aspect of the knee (inside part-where the knees touch).  Patient’s filled out detailed questionnaires on their final treatment and 79.8% of them reported that their knees were moderately to a great deal better.  It was concluded that low-level laser therapy or cold laser therapy for arthritis knee pain helps improve the overall quality of life for patients suffering with chronic knee pain.  The following is an excerpt from the abstract.

LOW-LEVEL LASER THERAPY AND ITS EFFECTS ON THE QUALITY OF LIFE FOR PATIENTS SUFFERING FROM OSTEOARTHRITIS KNEE PAIN IN THE MEDIAL COMPARTMENT: A DETAILED ANALYSIS BY QUESTIONNAIRE

Introduction:  The aim of this study was to assess the effects of low-level  laser therapy (cold laser therapy) for patients specifically diagnosed with moderate to severe (bone on bone) osteoarthritis knee pain in the medial compartment of the knee by using questionnaires. 

Methods:  Patients were treated at the DFW Spine & Joint Clinic from June 2010 thru April 2011.  There were a total of 119 knees treated representing 86 patients.  One hundred nineteen questionnaires were completed and collected after the patients treatment.

Results: There were a total of 86 patients diagnosed and treated for moderate to severe (bone on bone) osteoarthritis in the medial joint compartment with symptoms of knee pain.  This diagnosis was a result of either an x-ray or an MRI. Out of those patients 33 had both knees treated and 53 patients had only one knee treated.  There were a total of 34 males and a total of 52 females, with a mean age of 66.13 years.

 Patients were asked to rate their overall change in activity, limitations, symptoms, emotions, and quality of life after completing 20 treatments using a detailed questionnaire.  Patients who had both knees treated were asked to fill out two different questionnaires, one for each knee treated.   Overall 79.8% rated the overall improvement of their knee either moderately better to a great deal better.  None of the patients rated that the treatments had negative effects on their quality of life or that they experienced more knee pain on the completion of their 20th treatment.

Conclusion: Our results indicate that Low-level laser therapy (or cold laser therapy) significantly improves the overall quality of life and related factors for patients diagnosed with moderate to severe (bone on bone) osteoarthritis knee pain in the medial compartment of the knee making this therapy an ideal treatment for patients with chronic knee pain.

Consider Cold Laser Therapy if You’re Still Experiencing Knee Pain after Having Knee Surgery

Today an individual (I’ll name Mr. V to protect his privacy) came to see me because two months ago he had undergone arthroscopic knee surgery to correct a meniscus tear, but after the surgery he was still complaining of knee pain, swelling and the inability to fully extend his knee.  Unfortunately, Mr. V’s knee is no better today than prior to him having knee surgery.  In fact, his knee feels worse!

Even the x-rays reveal that his knee is now virtually ‘bone on bone’ (since most of his cartilage/meniscus was removed during knee surgery).Whereas before the knee surgery the x-rays indicated that he had cartilage in the medial aspect of the joint.   In a nutshell, his knee is structurally worse, and his symptoms are more severe since getting it scoped.

When Mr. V went back to the surgeon (by the way this surgeon works at one of the most prestigious clinics in North Dallas) to tell him his knee was not getting any better the surgeon told him to take three Alleve and deal with the pain, or undergo a series of gel injections.

Unfortunately, his orthopedic surgeon told him his knee would be ‘as good as gold’ after the surgery.  To make matters worse the surgeon didn’t recommend therapy after the surgery because he just ‘didn’t think it would make a difference’.

Well I beg to differ…

I think therapy after knee surgery is crucial, and I believe it would have made a HUGE DIFFERENCE in Mr. V’s outcome.  In fact, in his case I think therapy should have been recommended BEFORE knee surgery was ever even performed.

What happened to Mr. V’s knee ‘being as good as gold’ after the surgery?

Unfortunately, Mr. V’s case is not unique.  Every day I get frustrated, depressed patients coming in to see me because they are still having knee pain after arthroscopic knee surgery.   And instead of the surgeon recommending cold laser therapy to help decrease nerve irritation to the synovium (the lining of the bone that gets irritated and causes most of the pain) and to reduce inflammation in the surrounding tissues they tell them to either live with the pain, get an injection or get a knee replacement (because after the surgery they are suddenly ‘bone on bone’).   Instead the surgeons’ ego gets in the way of acknowledging that the patient needs to be referred to another doctor for a second opinion! Whatever happened to doing what’s best for the patient?!!

Not to toot my own horn…but I get great results with individuals who are still experiencing knee pain, swelling, and reduced range of motion after arthroscopic knee surgery.  The reason these individuals do so well with my Cold Laser Knee Program is because the knee is IRRITATED FROM THE SURGERY and cold laser therapy and the other specific modalities we use help reduce the irritation and inflammation in the knee joint.  In addition, we re-educate and strengthen the muscles surrounding the knee to help remove the stress on the knee joint.

Are You Living With ‘Bone On Bone’ Knee Pain Because You Don’t Think You Have Any Other Options Besides Knee Surgery? Think Again.

Most of the time patients complaining of ‘bone on bone’ knee pain are told that total knee replacements are their only option, and are never given any other alternatives in order to delay or avoid knee surgery. So rather than go under the knife many individuals just choose to ‘live with the pain.’

Just because you don’t want knee surgery doesn’t mean you have to live with ‘bone on bone’ knee pain. These days there are painless and less-invasive alternatives to surgery that may provide you with knee pain relief so that you can avoid or delay knee surgery. Why Cold Laser Therapy and Exercises Are Better Options Then Knee Surgery for Osteoarthritis Knee Pain

Nearly 27 million people suffer with osteoarthritis knee pain in the United States, and about 1 million arthroscopic knee surgeries are performed every year.  Arthroscopic knee surgery is a procedure where a scope and a small camera are inserted through a tiny incision.  Orthopedic surgeons use this procedure to remove damaged cartilage and remove loose bone chips in the knee.  Unfortunately, arthroscopic knee surgery leaves the knee with less cartilage which makes the knee vulnerable to future injuries, and arthritis. Because osteoarthritis in the knee worsens after surgery, and an individual’s symptoms don’t necessarily improve its effectiveness has been questioned.

Several studies published in the New England Journal of Medicine have indicated arthroscopic surgery should not be used for the treatment of osteoarthritis knee pain.  The first one was published in 2002 and found that arthroscopic knee surgery for the treatment of osteoarthritis knee pain was no better than a sham procedure.  The most recent study published in the September 2008 issue concluded that physiotherapy was as effective at relieving moderate to severe osteoarthritis knee pain as arthroscopic knee surgery.

In this recent study nearly 200 patients were randomly separated them into two groups.  The first group had physical therapy for three months to strengthen the muscles surrounding the knee joint.  In addition, the group was prescribed medications and supplements.  The medications included anti-inflammatories, Tylenol, glucosamine and chondroitin.  The other group had physical therapy, medications, and arthroscopic knee surgery.  After two years both groups reported the same pain levels, function and quality of life.

“This study offers hope and encouragement to persons with osteoarthritis who would like to avoid the pain and emotional toll of surgery, “says American Physical Therapy Association president R. Scott Ward.

Arthroscopic knee surgery for osteoarthritis knee pain “probably isn’t terribly helpful compared to just medication and physical therapy,” states Dr. C. David Geier Jr. a representative for the American Orthopaedic Society for Sports Medicine.

Christopher M. Powers, co-director of Musculoskeletal Biomechanics Research Lab at the University of Southern California added. “Many times knee pain is associated with abnormal movement patterns that cause increased stress on the joint.  Arthroscopic surgery does little to correct the dynamic factors that may be contributing to knee pain and pathology. These findings reinforce the need for a comprehensive treatment approach for such patients.”

This study reinforces my belief that cold laser therapy and exercises are better options than knee surgery for osteoarthritis knee pain. What Causes Osteoarthritis of the Knee?

Before I discuss what causes osteoarthritis of the knee let me review what healthy cartilage in the knee is made from. The cartilage in the knee joint is called hyaline cartilage or articular cartilage.  It is found at the ends of joints so that it allows for smooth movements.  It is made up of the following:

  • 70% Water
  • 15% Collagen (a protein).  Type II
  • 15% Proteoglycans (protein), Chondrocytes (cartilage forming cells), non collagen proteins, lipids and organic materials

In a healthy knee, cartilage is always in the process of being remodeled due to normal wear and tear of the knee joint. Hyaline cartilage is constantly being broken down by the enzyme, matrix metalloproteinase, and being replaced and rebuilt by chondrocytes (cartilage cells).  In a healthy knee there is a balance between cartilage cells that are rebuilding and replacing cartilage and enzymes that are breaking the cartilage down. Unfortunately, genetics, aging, weight, disease, surgery and injuries to the knee all play a role in contributing to throwing off that balance and allowing more cartilage to be broken down then regenerated.  When the amount of cartilage is being degraded faster than it is being synthesized by chondrocytes than cartilage naturally degenerates.

One of the main proteins that is believed to be responsible for causing this imbalance in enzyme activity and inhibiting cartilage development is interleukin-1.  This cytokine creates inflammation in the joint, it stimulates more of the enzyme that destroys cartilage, it does not allow collagen to be produced, and it suppresses chondrocytes from producing proteoglycans.

It is reasonable to assume that if interleukin-1 was inhibited, and the number of chondrocytes outnumbered the enzyme that destroyed cartilage then the effects of osteoarthritis would be negligible.

Before starting Dr. Schnee’s Cold Laser Knee Program I was informed that due to both knees being ‘bone on bone’ that I needed knee replacements. I was actually scheduled to have knee replacement surgery but I decided to complete Dr. Schnee’s Cold Laser Knee Program first to see if it would help relieve any of my knee pain.

Before starting treatments with Dr. Schnee, my left knee was sore and my right knee felt like there was an ice-pick jabbing into it on every step.   I could not get into my pool or work hardly at all due to the pain.  A few months ago, I even tried injections in both knees with no results.

Since completing Dr. Schnee’s Knee Program my knees feel 60% better, and I am no longer scheduled to get knee replacements.  Both of my knees have improved a lot and with the exercises Dr. Schnee gave me I hope to continue improving.
