Diagnosed With A Meniscus Tear in the Knee? It’s NOT the Reason You’re Having Knee Pain

Patients diagnosed with a meniscus tear in the knee often think that it’s the meniscus tear that is causing them to be in pain.  So they often undergo arthroscopic knee surgery to have the tear removed. Many times patients are disappointed after the surgery because the pain hasn’t completely gone away. Why?  This is because a meniscus tear is often blamed for knee pain when it really isn’t the cause.  Tears in the meniscus don’t actually cause pain because there are no nerve endings in meniscus tissue.  Instead, the pain is coming from the tissue and the lining around the joint (synovium) that is also irritated and inflamed and does have a nerve supply. In fact, YOU WON’T EXPERIENCE ANY KNEE PAIN EVEN WITH A MENISCUS TEAR if the nerves in the tissue or the lining around the joint aren’t inflamed or irritated.

The New England Journal of Medicine reported in September 2008 that meniscus tears on MRI’s are incidental findings and are very common in the “general population and increase with increasing age.”  Furthermore, 61% OF THE SUBJECTS IN THE STUDY HAD MENISCUS TEARS ON MRI BUT NONE OF THEM HAD PAIN, STIFFNESS OR ACHES.  THEY WERE SYMPTOM-FREE.  This study proves that MENISCUS TEARS DON’T CAUSE KNEE PAIN.  There are certain types of meniscus tears that may cause the knee to lock up and create problems with range of motion.  However, meniscus tissue has NO NERVE ENDINGS SO IT IS IMPOSSIBLE FOR YOU TO EXPERIENCE KNEE PAIN DIRECTLY FROM THE TEAR ITSELF.

I’ve treated hundreds of individuals diagnosed with knee pain from meniscus tears and I’ve been able to alleviate the pain, help them avoid arthroscopic knee surgery and keep them that way- even with the meniscus tear still there.  Initially individuals with a meniscus tear come into my office complaining of knee pain. The pain is often described as sharp and shooting (especially if they turn a certain way) or it can be deep, dull and achy.  In severe cases the pain prevents them from sleeping and walking for any length of time.  Under my care, a typical patient with a meniscus tear will complete a series of cold laser treatments along with physiotherapy modalities.  Usually after a few weeks of treatments they leave my office pain-free and the muscles around the joint are stronger.

What does my Cold Laser Knee Program do for patients with meniscus tears?  It relieves the pain and reduces swelling by eliminating nerve irritation and inflammation to the synovium (joint lining) and surrounding tissues. Cold lasers don’t CUT OUT OR REMOVE THE MENISCUS like arthroscopes (the tool used in arthroscopic surgery).  Why would you want to remove cartilage or meniscus if that’s not what’s causing your knee pain?  It won’t grow back if it’s removed and the area becomes a breeding ground for arthritis. At the DFW Spine & Joint Center we don’t want you to have to undergo unnecessary knee surgery.  Instead we want to use cold laser therapy to work on a cellular level to eliminate pain, reduce inflammation, increase blood flow, increase energy production of the cell, and, as a result, speed up the healing process.  In addition, to cold laser therapy we also want to strengthen the muscles around your knee joint so that it becomes more stable and your condition doesn’t reoccur.  Most patients who complete our Cold Laser Knee Program have sustainable results, and unless they reinjure their knee never have to see us for treatments in the future.

Mr. James S. is a classic example of a patient diagnosed with a medial meniscus tear.  He did not want arthroscopic knee surgery because it made sense to him that if he had the meniscus cut out it would NEVER grow back.  He also came into my office because he had educated himself about the meniscus.  He learned that the meniscus did not have a nerve supply so it was impossible for the pain to be coming from the tear. He also deduced that if he had arthroscopic knee surgery to remove the tear it would do nothing to reduce the irritation and inflammation for the tissue in the knee actually causing the pain.

The following is what Mr. James S. wrote about his knee before and after completing Dr. Schnee’s Cold Laser Knee Program “My knee pain was severe and I felt pain in the front and back of the knee. I couldn’t sleep because of the pain and I couldn’t stand for long.  Since completing Dr. Schnee’s Cold Laser Knee Program my knee is 90% better then it was and the best part is I can stand, walk and sleep through the night without experiencing knee pain.”

Why Cold Laser Therapy and Exercises Are Better Options Then Knee Surgery for Osteoarthritis Knee Pain

By Dr Schnee · September 24, 2010 · Filed in Knee Pain, Knee Surgery, Osteoarthritis · No Comments »

Nearly 27 million people suffer with osteoarthritis knee pain in the United States, and about 1 million arthroscopic knee surgeries are performed every year.  Arthroscopic knee surgery is a procedure where a scope and a small camera are inserted through a tiny incision.  Orthopedic surgeons use this procedure to remove damaged cartilage and remove loose bone chips in the knee.  Unfortunately, arthroscopic knee surgery leaves the knee with less cartilage which makes the knee vulnerable to future injuries, and arthritis. Because osteoarthritis in the knee worsens after surgery, and an individual’s symptoms don’t necessarily improve its effectiveness has been questioned.

Several studies published in the New England Journal of Medicine have indicated arthroscopic surgery should not be used for the treatment of osteoarthritis knee pain.  The first one was published in 2002 and found that arthroscopic knee surgery for the treatment of osteoarthritis knee pain was no better than a sham procedure.  The most recent study published in the September 2008 issue concluded that physiotherapy was as effective at relieving moderate to severe osteoarthritis knee pain as arthroscopic knee surgery.

In this recent study nearly 200 patients were randomly separated them into two groups.  The first group had physical therapy for three months to strengthen the muscles surrounding the knee joint.  In addition, the group was prescribed medications and supplements.  The medications included anti-inflammatories, Tylenol, glucosamine and chondroitin.  The other group had physical therapy, medications, and arthroscopic knee surgery.  After two years both groups reported the same pain levels, function and quality of life.

“This study offers hope and encouragement to persons with osteoarthritis who would like to avoid the pain and emotional toll of surgery, “says American Physical Therapy Association president R. Scott Ward.

Arthroscopic knee surgery for osteoarthritis knee pain “probably isn’t terribly helpful compared to just medication and physical therapy,” states Dr. C. David Geier Jr. a representative for the American Orthopaedic Society for Sports Medicine.

Christopher M. Powers, co-director of Musculoskeletal Biomechanics Research Lab at the University of Southern California added. “Many times knee pain is associated with abnormal movement patterns that cause increased stress on the joint.  Arthroscopic surgery does little to correct the dynamic factors that may be contributing to knee pain and pathology. These findings reinforce the need for a comprehensive treatment approach for such patients.”

This study reinforces my belief that cold laser therapy and exercises are better options than knee surgery for osteoarthritis knee pain. It’s always better to try conservative non-invasive treatments first before going under the knife! At the DFW Spine & Joint Center we combine cold laser therapy, along with other physiotherapy modalities and exercises to strengthen and rehabilitate the knee. Arthroscopic Knee Surgery to Repair ACL Injuries or Meniscus Tears Won’t Stop Osteoarthritis from Developing In the Knee

A study published in the journal of Radiology (August 2010 issue) followed 326 men and women 10 years after they had arthroscopic knee surgery to repair torn Anterior Cruciate Ligaments (ACL) and meniscus tears. Initially, researchers hypothesized that patients who had arthroscopic knee surgery would lower their risk for developing osteoarthritis in their knee and also lower their risk for other complications such as loss of cartilage, lesions in the bone marrow, and narrowing of the joint space.  However, this was not the case, osteoarthritis developed in the men and women’s knees’ despite doing knee surgery to fix the injuries.

This research study confirms what I view on x-rays on a daily basis which is knee surgery does NOT stop osteoarthritis from developing.  instead it actually helps contribute and speeds up the process of cartilage loss and deterioration!

The bottom line is knee surgery will not prevent arthritis in the knee, so don’t be fooled into thinking that it will…

Knee Pain After Arthroscopic Knee Surgery Reduces Activity Level for Patients

By Dr Schnee · February 19, 2010 · Filed in Knee Surgery, Meniscus Tear · No Comments »

A long-term study to determine the outcomes of patients who underwent arthroscopic  partial  lateral meniscectomy (partial removal of the lateral meniscus) for lateral meniscus tears was conducted in otherwise normal knees.  The study reviewed patient’s activity level, clinical, and x-ray results.

Patient’s who underwent the surgery were followed up with a minimum of 5-8 years later.  They were evaluated by questionnaire, x-rays, and knee examinations.  Initially, only 85% of those who underwent knee surgery were able to temporarily return to pre-injury activity level.  However, this percentage dropped to 48% at the time of follow-up.     The study concluded that the activity level of patients undergoing arthroscopic surgery for partial lateral meniscus removal is likely to deteriorate with time.  The study also concluded that patients should be counseled concerning long-term expectations regarding their knee pain.

If you’re still suffering from knee pain after undergoing arthroscopic knee surgery then Dr. Schnee’s Cold Laser Knee Program may be the answer to your problem.  Dr. Schnee’s Cold Laser Knee Program is considered a complete treatment program that combines low-level laser treatments (cold laser treatments), along with other non-invasive  modalities that promote rehabilitation of the knee.

Every patient’s program is different depending on whether they suffer from knee pain due to arthritis, ‘bone on bone’, meniscus tears, bursitis, tendonitis, Baker’s Cyst, post surgical arthroscopic knee pain, or a combination of the listed conditions.  If you live in the Dallas/Fort Worth area call our office at 214- 596-1051 to find out if you’re a candidate for Dr. Schnee’s Cold Laser Knee Program.

Arthroscopic Knee Surgery is Not A Cure-Most Have to Repeat the Procedure

By Dr Schnee · February 18, 2010 · Filed in Knee Surgery, Meniscus Tear · No Comments »

Yes, arthroscopic knee surgery does help some patients get relief from knee pain but on the other hand so do shots, and medications.   At least with shots and medications a doctor is not removing cushion from the knee joint as in the case of arthroscopic knee surgery. (Unfortunately, there are other side-effects that arise from getting shots and taking medications that I will discuss in another blog.)  After years of published research, we now know that IF arthroscopic surgery does help relieve knee pain, by removing the meniscus either partially or in its entirety,  the effects are often temporary in nature (a few months or years) and in the long run the surgery leaves the knee vulnerable to future tears and arthritic changes.

In a retrospective review of the results of  arthroscopic procedures that were performed for the treatment of an acute or a chronic meniscal lesion, with or without an associated ligamentous lesion, eighty meniscal tears were identified (in seventy-five patients) that had been assumed to be stable. Of the seventy-five patients, fifty-two had been followed for two to ten years. A repeat arthroscopy was performed on thirty-two patients at an average of twenty-six months after the original arthroscopy. This means that over 61% of patients who underwent arthroscopic knee surgery had to repeat it again because it was unsuccessful the first time.

Severe Osteoarthritic Changes Occur in Knees Over Forty, after Arthroscopic Knee Surgery

By Dr Schnee · February 17, 2010 · Filed in Knee Surgery, Osteoarthritis · No Comments »

I’ve written previous blogs about the subject of arthroscopic knee surgery, and in most cases I believe it is a useless procedure  because  in the long run it creates more osteoarthritic changes in the knee  which ultimately leaves the patient with the possibility of having to get a  total knee replacement later in life.  Multiple studies support this idea and clearly indicate that osteoarthritic changes become more prominent after arthroscopic knee surgery.  A recent study conducted at the Department of Traumatology in Austria also supports these studies.   In this current study, arthroscopic knee surgery was performed on 284 patients to remove part of a torn meniscus.

After the surgery was completed x-rays of the knees were taken.  Patients were then X-Rayed and evaluated again 53.5 months after the surgery.  Two hundred forty-seven patients had been treated for medial meniscus tears and 37 for lateral menisci tears. Preoperative radiographs were compared with those at follow up and were classified. The results were analyzed statistically.

Osteoarthritic changes were classified as being worse in 38% of the patients after medial and in 24% of the patients after lateral arthroscopic partial meniscectomy (removing part of the meniscus).   Patients who were older than 40 years of age and who had undergone arthroscopic partial medial meniscus removal had a significantly higher rate of osteoarthritis than patients who were younger than 40 years of age. The study concluded that partial medial or lateral meniscus removal leads to a significant increase of osteoarthritic changes, after arthroscopic surgery especially if you are over 40 years of age.

If you’re still suffering from knee pain months or years after undergoing arthroscopic knee surgery then Dr. Schnee’s Cold Laser Knee Program may be the answer to your problem.  Dr. Schnee’s Cold Laser Knee Program is considered a complete treatment program that combines low-level laser treatments (cold laser therapy treatments), along with other non-invasive modalities that help promote rehabilitation of the knee.

Study Proves If Your Over Forty Your Knee Will Be Worse Off After Undergoing Arthroscopic Knee Surgery

By Dr Schnee · February 16, 2010 · Filed in Knee Surgery, Osteoarthritis · No Comments »

Forty-nine patients who were more than forty years old when medial meniscectomy (arthroscopic surgery to remove the medial meniscus due to a tear) was performed returned for follow-up evaluation one to twelve years postoperatively. Pain was present in 62.2 % of patients who underwent knee surgery to ‘clean out’ degenerative tears (tears due to arthritis), and in 75% of those who underwent surgery to ‘clean out’ traumatic tears (tears due to a knee injury).

The medial (inner) joint space was significantly thinner in patients who had knee surgery to clean out degenerative tears versus those with traumatic tears.

The x-ray findings of osteoarthritis were significantly more severe on the side operated on.

The angle of the knee was relatively more ‘bow legged’ on the surgically treated knee (mean, 5.3 degrees) versus the knee that did not undergo knee surgery.

In conclusion, before committing to undergoing knee surgery make sure you try all conservative knee treatment options first! Numerous reputable, long-term studies indicate that knee surgery does NOT fix the problem and in many cases it promotes more arthritis knee changes and knee pain. Please search for other alternatives to treating your knee pain before undergoing knee surgery. According to Two Year Study Reported in the New England Journal of Medicine Arthroscopic Knee Surgery is Useless if You Suffer With Osteoarthritis Knee Pain

By Dr Schnee · January 25, 2010 · Filed in Knee Pain, Osteoarthritis · No Comments »

A study entitled A Randomized Trial of Arthroscopic Surgery for Osteoarthritis of the Knee was published in the New England Journal of Medicine. This two year, single-center, randomized, controlled trial of arthroscopic surgery in patients with moderate-to-severe osteoarthritis of the knee was conducted to find out if surgery was effective.

Patients were randomly assigned to surgical lavage and arthroscopic débridement together with optimized physical and medical therapy, or patients were assigned for treatment with physical and medical therapy alone. The outcomes were determined after a 2 year follow up by WOMAC scores (Western Ontario and McMaster Universities Osteoarthritis Index), short form-36, and physical component scores.

Of the 92 patients assigned to surgery, 6 did not undergo surgery. Of the 86 patients assigned to control treatment, all received only physical and medical therapy. Patients who received physical therapy and medical therapy alone actually had slightly better outcomes two years after the surgery then those who received arthroscopic knee surgery for osteoarthritis of the knee.

The study concluded that after 2 years analysis of the scores that arthroscopic surgery for osteoarthritis of the knee provided no more benefit than optimized physical and medical therapy.

Once again this study proves what conservative doctors have known all along. Non-invasive knee treatments are the solution to osteoarthritis knee pain.

This randomized study just followed this group of patients for two years. However, I bet that if researchers continued to check in with those patients who received arthroscopic knee surgery they would discover that those patients were worse off years later than those who opted for conservative care. Why? Because arthroscopic surgery ‘cleans out the knee’, and with patients who have very little cartilage to begin with, due to osteoarthritis, the surgery leaves them with less cartilage, and meniscus. As a result of the surgery the knee becomes more vulnerable to arthritis, and patients have a greater chance of having to receive knee replacements in the future.

I doubt that if you are a patient who has been diagnosed with arthritic knees you would want to take a chance with surgery. Especially if you knew that arthroscopic surgery would increase the chances of you having to receive total knee replacements later in life.

Dr. Schnee’s Cold Laser Knee Program is a conservative, non surgical, safe, effective treatment program that may help you avoid knee surgery. Dr. Schnee’s Cold Laser Knee Program is considered a complete treatment plan that combines cold laser treatments, along with other non-invasive modalities that help promote rehabilitation of the knee.

The American Academy of Orthopaedic Surgeons Does NOT Recommend Arthroscopic Knee Surgery for Osteoarthritis Knee Pain

By Dr Schnee · January 22, 2010 · Filed in Cold Laser Therapy, Knee Surgery, Osteoarthritis · 1 Comment »

The American Academy of Orthopaedic Surgeons Clinical Practice Guidelines clearly state, “We recommend against performing arthroscopy with debridement or lavage in patients with a primary diagnosis of symptomatic OA (osteoarthritis) of the knee.” 1

Why don’t orthopedic surgeons recommend ‘cleaning up’ the knee with arthroscopic knee surgery for osteoarthritis knee pain? Because it doesn’t work, and most patients feel worse after the surgery and eventually need total knee replacements!!!

Try conservative treatments first before you get cut on!! After all, knee surgery is always an option, but once surgery is performed there is no turning back.
Dr. Schnee’s Cold Laser Knee Program is a conservative, non surgical, safe, effective treatment program that may help you avoid knee surgery. Dr. Schnee’s Cold Laser Knee Program is considered a complete treatment propgram that combines cold laser treatments, along with other modalities that help promote rehabilitation of the knee.
