How to Treat Meniscus Tears and Why You Should Avoid Knee Surgery

By Dr Schnee · August 27, 2010 · Filed in Cold Laser Therapy, Knee Surgery, Meniscus Tear · 1 Comment »

Did you know that there are millions of individuals who walk around every day with meniscus tears and are completely asymptomatic (they feel no pain)?  Unfortunately, once pain is perceived and a meniscus tear is diagnosed most orthopedic surgeons will usually recommend arthroscopic knee surgery.   However, since knee surgery in patients over the age of 40 is often unsuccessful and it accelerates the arthritic process in the knee it should only be considered once all conservative measures have been exhausted.

There are several different types of meniscus tears and the location and size of the tear determines how it should be treated and whether the meniscus tear has the potential to respond to conservative care.

A meniscus tear is in one of these three zones:

Red Zone- The red zone is located in the outer one third of the meniscus.  This area has a rich blood supply therefore a meniscus tear in this region responds well to conservative care.  Many times a tear in this area will heal spontaneously if given a chance. The RICE method may be useful in this case.  (RICE stands for rest, ice, compression, and elevation.)Sometimes, the tissue needs a ‘jumpstart’ or an extra ‘push’ so it can heal faster.  Therefore, cold laser therapy is a great option for individuals with this type of tear since it helps stimulate more blood flow, decreases pain, and inflammation in the area allowing the tissues to heal and repair faster.  Try to avoid arthroscopic knee surgery if you have a tear in this region.

Red/White Zone- This type of tear extends from the red zone (the outer one third) into the inner two thirds of the meniscus or the white zone.  The white zone is an area that has no blood supply.  Knee surgery may not work because the meniscus may not heal after the surgery is performed so conservative care should be considered first.  The goal for conservative care (ie cold laser therapy) with this type of tear is to help individuals get out of pain, and reduce swelling and keep them that way.

White Zone- Again the white zone is located in the inner two thirds of the meniscus. If you have a tear located in this zone this is a tricky area to treat because it is avascular.  This means there is no blood supply to the meniscus in this area. Due to the lack of blood supply a torn meniscus in this zone is unlikely to heal with ANY TYPE of treatment including arthroscopic knee surgery.  The goal with conservative care such as cold laser therapy is to help provide the individual with pain relief and decrease the inflammation in the knee joint.

In summary, many individuals have tears AND NEVER EVEN KNOW IT! So it is possible to be asymptomatic and still have a meniscus tear! Oftentimes individuals are scared into surgery by their doctors.  They’re told that if they don’t have knee surgery that they’ll eventually develop arthritis in the knee.

What do you think arthroscopic knee surgery does to your knee?!!

Knee surgery guarantees that you will develop arthritis, and the arthritic process will be accelerated.  It actually compounds the same problem (arthritis) your doctor told you would be avoided by undergoing knee surgery.  Furthermore, if the meniscus or meniscus tear is cut out or removed your knee joint is left with less of a cushion and shock absorber.

The bottom line is that after any injury or surgery, arthritis is likely to develop.  At least with conservative care the meniscus is left intact therefore the shock absorber (meniscus) isn’t removed.

Of course there are cases where knee surgery cannot be avoided; however, if possible give  your knee an opportunity to heal without knee surgery.  Many times conservative care will allow an individual to become asymptomatic, and depending on the type of tear and its location the tear may heal on its own. Try the RICE method. In addition, visit a doctor that specializes in treating the knee joint with cold laser therapy which may help your knee heal faster by increasing blood flow, decreasing pain, and reducing inflammation in the knee.

The following is a case study about a patient who chose Dr. Schnee’s Cold Laser Knee Program to treat his severe knee pain and swelling from a meniscus tear.

Recently, Mr. B. age 54, a television producer from Lewisville Texas came in to the DFW Spine & Joint Center complaining of severe knee pain and swelling in his left knee.  He rated his knee pain a 6-8 on a pain scale of 1-10.  His knee pain prevented him from walking without a limp and climbing stairs.  He even complained of knee pain while he was driving, and he could not sleep through the night without tossing and turning.  His MRI stated he had a medial meniscus tear in the red/white zone and severe arthritis.  He already had arthroscopic knee surgery for the same condition on the same knee the year prior. But Mr. B did not want to undergo knee surgery for a second time so instead he chose to complete Dr. Schnee’s Cold Laser Knee Program.

After completing treatments, Mr. B is now able to walk without any knee pain and is able to walk without a limp 90% of the time.  He has more stamina in standing positions and is now able to climb and descend stairs without experiencing knee pain.  According to Mr. B his knee feels 75-80% better than it did before starting knee treatments and most importantly he was able to avoid knee surgery.

Another Dallas Resident is Now Able to Avoid Knee Surgery with Dr. Schnee’s Cold Laser Knee Program

By Dr Schnee · July 28, 2010 · Filed in Knee Surgery, Meniscus Tear, Testimonial · No Comments »

Before Cold laser treatments my knee pain was so severe that I couldn’t bend my knee, walk, sleep through the night, or work!  I was diagnosed with a meniscus tear, ‘bone on bone’ arthritis, and a ligament tear by an orthopedic surgeon.  I had tried knee injections and pain medications but neither of those things helped my knee pain.  I was told to do total knee replacements, but I opted out to try Dr. Schnee’s Cold Laser Knee Program.  Now since completing treatments my knee is 75% better then it was before treatments and I can walk without a limp; I am able to work comfortably, and I can sleep better!

Study Suggests Results from Therapy Are Just As Good As Knee Surgery for a Torn ACL

By Dr Schnee · July 23, 2010 · Filed in Cold Laser Therapy, Knee Pain, Knee Surgery · 2 Comments »

Unfortunately, every young athlete is at risk for tearing their anterior cruciate ligament or ACL.  Most of the time orthopedic surgeons and patients rush to knee surgery to reconstruct the ACL, but the problem is there are no long term research studies proving that surgery is better than therapy.  And even though there is no proof indicating that knee surgery has better outcomes then rehab alone there are approximately 200,000 ACL reconstructions performed and over 3 billion dollars  spent annually on this procedure in the United States.

Recently Swedish Researchers published a study in the New England Journal of Medicine proving that rehab alone is just as good as knee surgery for torn ACLs.  Researchers randomly assigned 121 young active adults with torn ACLs into two separate groups.  The first group ONLY underwent therapy to rehabilitate the knee with the OPTION to have knee surgery in the future.  The second group had therapy first but also had ACL reconstruction surgery.

Surprisingly, after two years the results were about the same in both groups when it came to pain and activities of daily living.  And most importantly 61% of the patients that were in the group that only had therapy did not choose to have knee surgery.

Sadly, since there is a lot of money to be made by surgeons and hospitals performing surgeries, this study will probably not make a difference in the number of ACL reconstruction’s performed annually. However, I hope that individuals with a torn ACL give therapy a chance and talk to their doctors about all non-surgical options available, including cold laser therapy, prior to undergoing ANY type of knee surgery.

Keep in mind, surgery is always an option, but should only be considered once all other options have been exhausted.

Knee Pain Sufferers Consider Knee Injections Painful and Ineffective Compared to Cold Laser Therapy for Treating Knee Pain

Most patients suffering from chronic knee pain have tried at least one cortisone injection and a series of SYNVISC™ or some other type of gel injections with little to no long term results.  The most common injection given to patients with knee pain is a steroid or cortisone shot.  Cortisone injections usually work to help decrease pain and inflammation but the results are often temporary.  Since there are long term side effects with the use of cortisone injections patients are limited to three injections per year. Gel injections are also available such as SYNVISC™, Hyalgen™, Euflexxa™, Orthovisc™, Supartz™ (otherwise known as cox comb, or rooster comb injections). These injections are usually performed in a series over the course of a few weeks, and just like cortisone injections gel injections only ‘cover up’ or treat  the symptoms not the  cause of the problem.  Knee pain sufferers consider gel injections painful and ineffective compared to cold laser therapy for treating knee pain.

Recently, Linda P., a  realtor from North Richland Hills completed Dr. Schnee’s Cold Laser Knee Program in Irving, Texas after initially complaining of very severe knee pain. The following is what she said about her experience with knee injections versus cold laser treatments.

“I sought medical help [from an orthopedic doctor for my knee] and was first given a steroid injection [cortisone injection] in the knee and later I was given a series of cox comb shots.  THEY WERE EXTREMELY PAINFUL.  I felt relief, but it wasn’t permanent and certainly not as effective as the cold laser treatments have been.  My medical doctor never recommended knee surgery because I was not a candidate for it.  The only solution I was given (which was not even a solution) was to have a series of cock’s comb shots twice a year and to just learn to live with the knee pain.  I am so pleased that I completed Dr. Schnee’s Cold Laser Knee Program.  I am 100% better and I intend to keep coming back so that I can stay that way!”

Linda’s experience is not unique.

Jill V., 62 years young, from Plano Texas is another example of a patient who recently completed cold laser therapy for knee pain with Dr. Schnee.  Before coming in for treatments with Dr. Schnee she tried cortisone injections, physical rehabilitation, a knee brace, and a series of three Euflexxa injections.  Ms. V. stated, “I had zero results from the knee injections.  The Euflexxa did not work for me, the pain never lessened.” She continued to say, “[Since completing Dr. Schnee’s Cold Laser Knee Program I am walking without knee pain, I don’t wake up at night whenever I move my leg.  There are times I even forget my knee ever hurt so badly.  [Dr. Schnee’s Cold Laser Program] has helped me get back on track.  Anyone who has knee issues, needs this program.”

If you have had knee injections including cortisone, SYNVISC™, Othovisc™, Euflexxa™, Supartz™, Hyalgen™, rooster comb, cox comb etc. and they did not work for you then call our office at 214-596-1051 to find out if you are a candidate for Dr. Schnee’s Cold Laser Knee Program at the DFW Spine & Joint Center in Irving, Texas.

Do you have ‘bone on bone’ knee pain and want to avoid or are not a candidate for total knee replacement surgery?

There is a large subgroup of patients 55 years of age and older who have been diagnosed as having ‘bone on bone’ or severe degenerative arthritis in their knees, and are either not candidates for knee surgery or do not want to undergo total knee replacement surgery.  Many of these patients either feel they are too young and the replacement won’t last; concerned with the risks associated with knee surgery; in poor health and are unable to have surgery; or unwilling to do the rehab necessary after the surgery is performed.   Usually patients suffering with ‘bone on bone’ knee pain or arthritis knee pain regard knee replacement surgery as their final option and are thrilled if they experience a 50-90% improvement in knee pain relief with non-surgical options such as cold laser therapy.

Mrs. Nikki G., from Carrollton Texas ,is a good example of someone who was diagnosed by an orthopedic surgeon as having ‘bone on bone’ knee pain but after completing Dr. Schnee’s Cold Laser Knee Program is now virtually pain free and will avoid knee surgery.

“I visited an orthopedic surgeon twice [before visiting with Dr. Schnee] because everyday activities were a hardship.  For awhile I had to use crutches but it was just impossible [to get around].  Everything seemed twice as hard to do.  My knee hurt most of the time especially when walking, standing and stair climbing.  It was difficult to straighten my knee making it difficult to sleep comfortably.  The orthopedic surgeon suggested that I start out trying anti-inflammatories and physical therapy.  He said that if those options didn’t work then I would have to have shots and then [knee] surgery.  I did some physical therapy but was not happy with the progress. [ So I finally decided to do research on non-invasive knee treatments on the internet and I found Dr. Schnee’s Cold Laser Knee Program].  Since completing care with Dr. Schnee I can go about my normal activities with ease and I can finally sleep at night without knee pain.  I am very pleased with my treatment and the results from cold laser therapy.  I feel like my knee is about 80% improved!”

Are there patients who don’t realize any improvements after completing Dr. Schnee’s Cold Laser Knee Program?  Yes, just as with any other treatment, there is a small percentage of patients who may not realize any improvement at all.  However, the good news is that are no side effects from treatments.

In addition, Dr. Schnee has two policies which she strictly adheres to preventing her from wasting anyone’s time.  First, if she doesn’t believe that she can help a patient she will not accept their case. Second, she will not start care with a patient whose expectations are unreasonable.   For example, a patient suffering with significant cartilage loss with the goal of running a marathon would not be considered a candidate for the Cold Laser Knee Program.  On the other hand, the same patient with the simple goal of significantly less knee pain, better joint mobility, a reduction in swelling and an overall better quality of life would be considered an ideal candidate for care.

If you are suffering from ‘bone on bone’ knee pain or severe degenerative arthritis in the knee and want to avoid or are not a candidate for knee surgery, and would be thrilled with a 50%-90% improvement in knee pain, reduction in swelling, and increased joint mobility then consider Dr. Schnee’s Cold Laser Knee Program as an option.

Arthroscopic Knee Surgery to Repair ACL Injuries or Meniscus Tears Won’t Stop Osteoarthritis from Developing In the Knee

A study published in the journal of Radiology (August 2010 issue) followed 326 men and women 10 years after they had arthroscopic knee surgery to repair torn Anterior Cruciate Ligaments (ACL) and meniscus tears. Initially, researchers hypothesized that patients who had arthroscopic knee surgery would lower their risk for developing osteoarthritis in their knee and also lower their risk for other complications such as loss of cartilage, lesions in the bone marrow, and narrowing of the joint space.  However, this was not the case, osteoarthritis developed in the men and women’s knees’ despite doing knee surgery to fix the injuries.

This research study confirms what I view on x-rays on a daily basis which is knee surgery does NOT stop osteoarthritis from developing.  instead it actually helps contribute and speeds up the process of cartilage loss and deterioration!

The bottom line is knee surgery will not prevent arthritis in the knee, so don’t be fooled into thinking that it will…

“I had knee pain that was almost unbearable.  I couldn’t stand or sleep in bed without having severe knee pain.  I had to sleep in a recliner because my knee hurt.  I tried cortisone shots and rooster comb or gel injections but I didn’t get any knee pain relief  from those.  I was told that I needed a knee operation, but I had too many friends that had one done and they were still hurting so I didn’t want to do knee surgery.

I’ve almost completed my cold laser treatments with Dr. Schnee, and I am very pleased with my results.  I can now sleep in my bed with very little knee pain. My knee is 90% better then it was prior to completing treatments with Dr. Alexandra Schnee!”

Patient Cancels Knee Replacement Surgery After Bone on Bone Knee Pain is Eased with Cold Laser Knee Program in Irving, Texas

By Dr Schnee · June 22, 2010 · Filed in Knee Surgery, Testimonial · No Comments »

Before starting Dr. Schnee’s Cold Laser Knee Program I was informed that due to both knees being ‘bone on bone’ that I needed knee replacements. I was actually scheduled to have knee replacement surgery but I decided to complete Dr. Schnee’s Cold Laser Knee Program first to see if it would help relieve any of my knee pain.

Before starting treatments with Dr. Schnee, my left knee was sore and my right knee felt like there was an ice-pick jabbing into it on every step.   I could not get into my pool or work hardly at all due to the pain.  A few months ago, I even tried injections in both knees with no results.

Since completing Dr. Schnee’s Knee Program my knees feel 60% better, and I am no longer scheduled to get knee replacements.  Both of my knees have improved a lot and with the exercises Dr. Schnee gave me I hope to continue improving.
