Only 48% Were Able to Return to Their Normal Activities, Several years After Arthroscopic Surgery

A long-term study to determine the outcomes of patients who underwent arthroscopic  partial  lateral meniscectomy (partial removal of the lateral meniscus) for lateral meniscus tears was conducted in otherwise normal knees.  The study reviewed patient’s activity level, clinical, and x-ray results.

Patient’s who underwent the surgery were followed up with a minimum of 5-8 years later.  They were evaluated by questionnaire, x-rays, and knee examinations.  Initially, only 85% of those who underwent surgery were able to temporarily return to pre-injury activity level.  However, this percentage dropped to 48% at the time of follow-up.     The study concluded that the activity level of patients undergoing arthroscopic surgery for partial lateral meniscus removal is likely to deteriorate with time.  The study also concluded that patients should be counseled concerning long-term expectations regarding their knee pain.

Is Arthroscopic Knee Surgery a ‘cure all’? Not if 32 out of 53 patients had to repeat the procedure…

Yes, arthroscopic surgery does help some patients get relief from knee pain but on the other hand so do shots, and medications.   At least with shots and medications a doctor is not removing cushion from the knee joint as in the case of arthroscopic knee surgery. (Unfortunately, there are other side-effects that arise from getting shots and taking medications that I will discuss in another blog.)  After years of published research, we now know that IF arthroscopic surgery does help relieve pain, by removing the meniscus either partially or in its entirety,  the effects are often temporary in nature (a few months or years) and in the long run the surgery leaves the knee vulnerable to future tears and arthritic changes.

In a retrospective review of the results of  arthroscopic procedures that were performed for the treatment of an acute or a chronic meniscal lesion, with or without an associated ligamentous lesion, eighty meniscal tears were identified (in seventy-five patients) that had been assumed to be stable. Of the seventy-five patients, fifty-two had been followed for two to ten years. A repeat arthroscopy was performed on thirty-two patients at an average of twenty-six months after the original arthroscopy. This means that over 61% of patients who underwent arthroscopic knee surgery had to repeat it again because it was unsuccessful the first time.

More Severe Arthritis, Pain and Bow-legged if Over Forty and had Arthroscopic Knee Surgery

Forty-nine patients who were more than forty years old when medial meniscectomy (arthroscopic surgery to remove the medial meniscus due to a tear) was performed returned for follow-up evaluation one to twelve years postoperatively. Pain was present in 62.2 % of patients who underwent surgery to ‘clean out’ degenerative tears (tears due to arthritis), and in 75% of those who underwent surgery to ‘clean out’ traumatic tears (tears due to injury).

The medial (inner) joint space was significantly thinner in patients with degenerative tears versus those with traumatic tears.

The x-ray findings of osteoarthritis were significantly more severe on the side operated on.

The angle of the knee was relatively more ‘bow legged’ on the surgically treated knee (mean, 5.3 degrees) versus the knee that did not undergo surgery.

Knee Surgery Should Always Be the LAST Option!

After consulting with hundreds of patients who have undergone unsuccessful arthroscopic surgery and knee replacements I’m more convinced  than ever that patients need to try conservative treatments prior to undergoing knee surgery.   The wonderful thing about conservative, non- invasive treatments such as cold laser, exercise, massage therapy, and electrical stimulation is that body parts are not being removed.   These treatments won’t deform your knee and are not likely to make your condition worse. Most importantly death is not a side-effect that you have to worry about.

For instance in the case of my  Cold Laser Knee Program where I combine multiple cold lasers simultaneously, non weight bearing exercises, and other modalities an average of eight out of ten patients  respond to care.  The average patient is usually able to achieve between a 50%-100% improvement in pain and function depending on the severity of the condition being treated.  Respectively, two out of 10 patients do not respond to care, and have no change in symptoms or function.

In the case of cold laser treatments, the worst case scenario would be that a patient doesn’t respond to care at all.   Compare that worst case scenario to undergoing unsuccessful knee surgery.  In the case of unsuccessful knee surgery parts of the knee have been removed permanently.  If the surgery is a failure there is no ‘going back’.  If arthroscopic surgery is unsuccessful often times either more arthroscopic surgery or a total knee replacement is recommended.

Obviously there is no comparison between surgery and  cold  laser  treatments  yet I’m still  surprised at the number of people I talk to on a daily basis that skip basic, conservative non invasive care and jump right into knee surgery.  I often ask patients that have been through unsuccessful arthroscopic surgery or knee replacement surgery why they didn’t receive conservative care BEFORE surgery was performed.  Often times their doctor told them that surgery was their only option, and didn’t offer them any other alternatives.

These patients are often willing to try ‘anything’ because they have ‘nothing to lose’ and are desperate to find a solution to their knee pain. Unfortunately, they have nothing to lose because by the time they’ve gone through knee replacements or arthroscopic knee surgery they’ve already ‘lost it’ (bone, cartilage, meniscus etc…).

Fortunately, there is hope for patients suffering with knee pain after arthroscopic or knee replacement surgery.  They are often great candidates for our Cold Laser Knee Program.   They are more challenging to treat, but typically respond very well to care.

Consider everything you have to ‘lose’ before you ‘lose it’ and give conservative care a chance BEFORE getting cut into.

The American Academy of Orthopaedic Surgeons Does NOT Recommend Arthroscopic Knee Surgery for Osteoarthritis Knee Pain

The American Academy of Orthopaedic Surgeons Clinical Practice Guidelines clearly state, “We recommend against performing arthroscopy with debridement or lavage in patients with a primary diagnosis of symptomatic OA (osteoarthritis) of the knee.” 1

Why don’t orthopedic surgeons recommend ‘cleaning up’ the knee with arthroscopic knee surgery for osteoarthritis knee pain? Because it doesn’t work, and most patients feel worse after the surgery and eventually need total knee replacements!!!

Try conservative treatments first before you get cut on!! After all, surgery is always an option, but once surgery is performed there is no turning back.

No, not according to Georgetown University Department of Orthopedics Study!

Study Conducted at Georgetown University Department of Orthopedics, 3800 Reservoir Road, Washington, DC 20007, USA.:

The study was designed to determine if patients clinically diagnosed with degenerative medical meniscus tears would significantly improve if they did not  elect to do surgery and chose nonoperative-based treatment protocols. Twenty-six patients were followed for 37 months and evaluated clinically, radiographically, and with standardized, validated Modified Lysholm Knee Scoring System (MLKS) and Standardized Activities of Daily Living Scale of the Knee (SADL) questionnaires. Forty-six percent of patients (12 of 26) declined operative treatment.

AFTER  A  3  YEAR FOLLOW UP 100% OF THE PATIENTS THAT RECEIVED NONOPERATIVE TREATMENT  IMPROVED TO A FUNCTIONAL LEVEL THAT ALLOWED THEM TO AVOID SURGERY. Arthroscopic Knee Surgery Actually Contributes to ‘Bone on Bone’

surgeryA newer study published in Arthritis and Rheumatism suggests arthroscopic procedures might actually contribute to cartilage degeneration. This includes patients that undergo arthroscopic surgery for meniscus tears and arthritis.  If the meniscus is shaved or removed it doesn’t grow back. As a result, arthritis will likely occur in the knee joint. In addition, when arthroscopic surgery is performed on an arthritic knee to ‘clean it out’ cartilage is removed.  The outcome leaves the patient with less cartilage that won’t grow back, and more arthritic changes eventually occur in the knee.

Whether a patient undergoes an arthroscopic procedure for arthritis or for a meniscus tear it leaves a patient vulnerable to arthritic changes in the knee which unfortunately may lead to knee replacements later in life.

I had Arthroscopic surgery on both knees (for cartilage tears) which caused Arthritis behind my knee caps. Arthroscopic Surgery for Arthritis Knee Pain is Proven Worthless

schnee workingPrior to receiving treatment in my office, half of my patients have had at least one or more arthroscopic knee surgeries, which did not meet their expectations. Unfortunately, after the surgery many of these patients still suffered from knee pain, swelling, reduced range of motion or some other problem that arthroscopic surgery did not fix. If only those patients would have known about the double blind studies that the New England Journal of Medicine had published about the results of arthroscopic surgery. The information would have saved them months of unnecessary suffering…

The New England Journal of Medicine, after two years of follow-up, reported on the results of a randomized, placebo-controlled trial to evaluate the efficacy of arthroscopy for osteoarthritis of the knee. It was concluded that patients who had arthroscopic surgery (lavage or debridement) performed were no better off than those who underwent the placebo (fake surgery) procedure. (1) Dr. Kenneth Fine, an orthopedic surgeon at the George Washington University School of Medicine, said the procedure had long seemed to do nothing for patients’ “underlying arthritis”. (2)

As a matter of fact arthroscopic surgery actually creates more of the same problem that it is trying to repair. ‘Cleaning out’ or taking out cartilage just leads to more arthritis in the future. Unfortunately, this leaves the knee vulnerable to becoming ‘bone on bone’ and eventually a knee replacement must be performed.
