National Institutes of Health: Proper Knee Alignment May Prevent Arthritis Knee Pain

The National Institutes of Health recently conducted a research study on knee alignment and how it affects the knee joint.  A research team followed 2,713 volunteers from Birmingham and Iowa City.  The volunteers were between the ages of 50-79.  All of them either had arthritis in their knees or they were considered at high risk for developing arthritis because they were either overweight, had a previous knee injury, or had undergone previous knee surgery.

First, X-rays were taken and knee alignment was measured.   The alignment was measured and the angle was noted at the point the femur (upper leg) and tibia (lower leg) intersected at the knee joint.  Anything more or less than a 2 degree difference from the normal 180 degree was noted.  X-rays and measurements were taken again 2 years later.

The research team discovered that in volunteers whose knee alignment measured a 2 degree difference (more or less) there was a 1.5 times greater risk for developing arthritis compared to those with a normal (180 degree) alignment.

In a healthy knee 70% of all the force is placed on the medial aspect of the knee (inside). If  the knee joint is out of  alignment the stress on the inside of the knee increases which in turn wears down the inside of the knee at a faster rate.  The NIH research team concluded that if a misaligned knee is corrected early on arthritis could be prevented.

To correct poor alignment of the knee visit a chiropractor, and wear good shoes that provide proper support.  An orthotic may be necessary if you are flat footed or if you have too high of an arch.  In addition, a chiropractor who specializes in the knee joint can give you proper exercises to strengthen and provide support for proper knee alignment.

Poor Mental Health May Be Making Your Osteoarthritis Knee Pain Worse

According to researchers at UC Davis School of Medicine osteoarthritis knee pain is directly related to an individual’s state of mental health.  The study was conducted by Dr. Wise, a researcher from UC Davis, and his colleagues. They were curious about why osteoarthritis knee pain sufferers reported day to day variations in the intensity of osteoarthritis knee pain when there were no actual structural changes to the knee. The study concluded that people with better mental health felt less pain then those with poor mental health.

Dr. Wise stated, “Pain varies over time, both over extended periods and over shorter periods. The same person can feel little or no pain in their knee or hip and later they can feel moderated to severe pain even when the extent of damage to the knee or hip joint as seen on x-ray imaging remains the same.”

The study found that a mental health score of 28-30 was associated with less osteoarthritis pain.  A score of 13-22 was associated with a higher pain scale.  Patients scoring between 13-22 had double the risk of increased pain flare ups as compared to participants in the 28-30 range.

Besides non-invasive treatments such as vitamins, supplements, exercise, and cold laser therapy:  mental health treatment may be another important factor in improving osteoarthritis knee pain.

Dallas Cold Laser Therapy for Knee Pain: What to do if you have severe knee pain

Severe knee pain affects millions of people annually in the United States and many individuals just deal with the pain rather than visit their doctor to get the appropriate treatment.  Unfortunately, there are conditions that need to be treated otherwise they will never heal or improve on their own.   For instance, certain types of meniscus and ligament tears, arthritis, bone on bone, chondromalacia patella; these conditions require the individual to be proactive otherwise the condition worsens.  Many times a patient’s tolerance to pain increases to the point that they don’t even notice how much pain they are in until they can hardly walk unassisted or climb stairs anymore.

There are some basic things patients generally can do on their own to help decrease the pain and swelling.  First, if you have swelling around the knee joint DON’T USE HEAT* to reduce the inflammation!  I understand that heat feels good when it’s being used but many people will complain of more pain and swelling hours after they’ve used it.  Instead, use an ice pack for twenty minutes every two hours around the joint.  Make sure you place a hand towel between the ice pack and the skin so you don’t get FREEZER BURN!  And don’t use the ice pack more than twenty minutes because ‘more is not necessarily better’.

Second, if exercise causes you to have knee pain STOP EXERCISING because you may be further damaging the joint!! Most people follow the old adage ‘no pain- no gain’.  This is untrue when it comes to the knee joint especially if you are suffering with severe knee pain.   You actually may be making your condition worse if you are exercising through the pain! The only time exercise is appropriate is when the condition is not severe or acute, if the knee joint is not irritated or damaged, and if exercise helps relieve the pain.

Most patients that begin an exercise regime over the age of 50 that have been diagnosed with severe arthritis and bone on bone are at risk of tearing the meniscus or cartilage in their knee joint.  The reason for this is because over time arthritis dries up the cartilage and meniscus in the knee.  For example, visualize a wet sponge.   It’s nice and plump, pliable and not easily torn.  On the other hand, a dry sponge loses its flexibility, cracks, and tears very easily.  As a result, there is a higher prevalence of meniscus and cartilage tears with older patients.

Finally, you shouldn’t have to suffer with severe knee pain.  Visit your health care provider to find out what’s causing the pain so that you can find a solution for it.  If you’ve already been to your doctor and you’ve been taking medications, knee injections or if you’ve gone through knee surgery and you’re still hurting then there are other options available to you.  Try cold laser therapy treatments combined with specific physiotherapy to address your knee pain and you’ll probably wish you did treatments sooner. *Please note any doctor can own cold laser equipment but it doesn’t make them proficient on the subject.  Make sure that you choose a doctor that focuses primarily on knee joints and that specializes in cold laser therapy.

Study Suggests Results from Therapy Are Just As Good As Knee Surgery for a Torn ACL

Unfortunately, every young athlete is at risk for tearing their anterior cruciate ligament or ACL.  Most of the time orthopedic surgeons and patients rush to knee surgery to reconstruct the ACL, but the problem is there are no long term research studies proving that surgery is better than therapy.  And even though there is no proof indicating that knee surgery has better outcomes then rehab alone there are approximately 200,000 ACL reconstructions performed and over 3 billion dollars  spent annually on this procedure in the United States.

Recently Swedish Researchers published a study in the New England Journal of Medicine proving that rehab alone is just as good as knee surgery for torn ACLs.  Researchers randomly assigned 121 young active adults with torn ACLs into two separate groups.  The first group ONLY underwent therapy to rehabilitate the knee with the OPTION to have knee surgery in the future.  The second group had therapy first but also had ACL reconstruction surgery.

Surprisingly, after two years the results were about the same in both groups when it came to pain and activities of daily living.  And most importantly 61% of the patients that were in the group that only had therapy did not choose to have knee surgery.

Sadly, since there is a lot of money to be made by surgeons and hospitals performing surgeries, this study will probably not make a difference in the number of ACL reconstruction’s performed annually. However, I hope that individuals with a torn ACL give therapy a chance and talk to their doctors about all non-surgical options available, including cold laser therapy, prior to undergoing ANY type of knee surgery.

Keep in mind, surgery is always an option, but should only be considered once all other options have been exhausted.

Do You Suffer With Knee Pain On The Inside Area Of The Knee Where The Knees Touch?

The majority of patients who experience knee pain have it on the inside area of the knee where the knees touch (medial joint line).  Many times this type of knee pain is due to cartilage loss on the inside of the knee, or a meniscus or cartilage tear.

Z. W., a retiree in Dallas, Texas is a perfect example of a patient who came to see Dr. Alexandra Schnee, D.C complaining of burning pain in the medial aspect of her knee and in the back of her knee.  Her knee pain had become so severe that it kept her from sleeping at night because every time she turned over in bed and her knees touched, she would wake up from the pain.  She also felt pain when she walked that was so intense, it make her feel ‘sick’.

Do you have ‘bone on bone’ knee pain and want to avoid or are not a candidate for total knee replacement surgery?

There is a large subgroup of patients 55 years of age and older who have been diagnosed as having ‘bone on bone’ or severe degenerative arthritis in their knees, and are either not candidates for knee surgery or do not want to undergo total knee replacement surgery.  Many of these patients either feel they are too young and the replacement won’t last; concerned with the risks associated with knee surgery; in poor health and are unable to have surgery; or unwilling to do the rehab necessary after the surgery is performed.   Usually patients suffering with ‘bone on bone’ knee pain or arthritis knee pain regard knee replacement surgery as their final option and are thrilled if they experience a 50-90% improvement in knee pain relief with non-surgical options such as cold laser therapy.

Arthroscopic Knee Surgery to Repair ACL Injuries or Meniscus Tears Won’t Stop Osteoarthritis from Developing In the Knee

A study published in the journal of Radiology (August 2010 issue) followed 326 men and women 10 years after they had arthroscopic knee surgery to repair torn Anterior Cruciate Ligaments (ACL) and meniscus tears. Initially, researchers hypothesized that patients who had arthroscopic knee surgery would lower their risk for developing osteoarthritis in their knee and also lower their risk for other complications such as loss of cartilage, lesions in the bone marrow, and narrowing of the joint space.  However, this was not the case, osteoarthritis developed in the men and women’s knees’ despite doing knee surgery to fix the injuries.

This research study confirms what I view on x-rays on a daily basis which is knee surgery does NOT stop osteoarthritis from developing.  instead it actually helps contribute and speeds up the process of cartilage loss and deterioration!

The bottom line is knee surgery will not prevent arthritis in the knee, so don’t be fooled into thinking that it will…

Severe Arthritis Knee Pain in Both Knees is Finally Relieved with Dr. Schnees’ Cold Laser Knee Program
