Exercises to Relieve Arthritis Knee Pain by Dallas Knee Doctor

Unfortunately, knee osteoarthritis causes stiffness and pain in the knee joints.  In order to help relieve arthritis knee pain it is important that you get moving.  Motion helps increase synovial fluid which helps the natural lubrication in your knees.  In addition, exercises strengthen the muscles around the knee which alleviates the pressure on the cartilage.

Unfortunately, not all exercises will help relieve knee pain.  As a matter of fact some exercises may aggravate your condition.  Here are some guidelines to follow:

Cycling is always the best option to help relieve knee pain:

Walking, cycling, and swimming will all help relieve mild osteoarthritis pain, and stiffness in the knees.  However, cycling or using a recumbent bike, in my opinion, is the best choice of activity for the knee joint.  A recumbent bike allows the thigh muscles to get strengthened without wearing down the cartilage in the knee joint.  Start by cycling for 5 minutes daily for one week, and every week thereafter increase your time by 5 minutes until you reach 20 minutes.  In the beginning, only use a resistance of 1 or 2,  and then continue to increase the resistance by one notch every three weeks until you reach a 5. Finally, make sure that the seat on the bike is adjusted properly so that you don’t further injure your knee joints.

Exercise done correctly should not cause any knee pain:

You should never experience any knee pain during or after exercise,  and if you do- STOP.  Please remember there is a significant difference between PAIN and soreness.  Experiencing muscle soreness after exercising is normal and is a good thing! It means you’re actually using and working out the muscles!!

Low weight, more repetitions:

If you’re doing leg extensions or leg curls at the gym make sure that you are using very light weight and completing more repetitions.  When performing a leg extension never stretch out the knee completely.  You should stop the extension right before the leg becomes fully extended, so the joint never locks.

Avoid doing squats and lunges:

Individuals with mild to severe osteoarthritis should not be doing squats or lunges; especially if you have been diagnosed with chondromalacia patella.  (This is a condition where the cartilage behind the knee cap has thinned and worn down.)

Avoid exercises that force the knee joint to ‘twist’.

Any activity where an individual is planting their foot on the ground, and twisting their upper body should be avoided.  This type of movement will aggravate an arthritic knee and may cause meniscus or cartilage tears in the knee.  The more arthritic the knee joint, the drier the cartilage becomes and any sort of twisting on the joint can cause meniscus tearing.  Individuals with moderate to severe arthritis should avoid playing golf and tennis because these activities will aggravate an arthritic knee.

Finally, keep in mind, ‘rest, will make you rust.’

If you suffer with moderate to severe osteoarthritis you will not feel like walking, cycling OR swimming.  However, it is vital that you get moving otherwise your condition will worsen. Doing five minutes of SOMETHING is better than doing NOTHING.  Once you start exercising, it will eventually become addictive because ALL of your joints will feel stronger and more flexible.

In conclusion:

Again, exercising the knee joint should not be torture! Your knee should feel good during and after any activity.  If it doesn’t then you’re doing the wrong exercise or you’re not doing the exercise correctly.

A report published online on July 7th, 2011 in Arthritis Care & Research by researchers at the University of Southampton in the United Kingdom found that women who gained weight over a fifteen year period had greater knee pain in both knees.  The study followed 594 females body mass index over a period of fifteen years and discovered that an increase in body mass index was associated with greater knee pain in both knees regardless of the severity of osteoarthritis diagnosed on the x-rays. 

Basically, this study reported on something women already intuitively know… that is the greater the weight gain the more stress it places on the knees.

Four Things to Help Relieve Runner’s Knee Pain

Stretch your hamstrings and calves every day. The best way to do this is to sit on a hard surface with your legs straight out in front of you and bend forward from your waist and try to touch your toes.  Hold for 45 seconds to a minute.  Repeat 5 times.

Roll out your Iliotibial band with a foam roller. The iliotibial band is a band of fascia that extends from the outside of the pelvis to below the knee joint.   You can purchase a foam roller from any sporting good facility.  The foam roller in the picture below is the more advanced version because it has ‘bumps’ on it.  I would recommend just starting with a smooth foam roller.  ‘Rolling out’ the IT band is painful because you’re essentially helping alleviate adhesions that have developed in the fascia.  This stretching routine is not for individuals that cannot tolerate pain well.  But afterwards you will certainly feel the difference.

Do side squats with a resistance band. Strengthening the hips will reduce knee pain and prevent future knee problems.

-Tie a resistance band around your ankles.  Stand with your feet shoulder width apart and squat.  Side step with your right leg as far as you comfortably can.  Bring left leg back to starting position. Do 12 steps to the right and then reverse.

Exercises Prevent Knee Injuries and Reduce Soldier Drop Out Rate in UK

Soldiers are expected to endure a strict exercise regime over the course of their training.  In the UK over 25% of British soldiers drop out of the army because of severe knee pain.  As a result, a new stretching and exercise routine was studied to discover whether it would lower the risk of knee injuries therefore reduce the dropout rate.  The study which was conducted over a 3.5 month period, with 1,500 soldiers found that certain exercises which included stretches and strengthening exercises for the major leg muscles reduced knee injuries by 75%.

Researchers concluded that targeted exercises for the leg muscles have helped reduce the risk of knee injuries; therefore, the dropout rates specifically due to knee pain complaints have drastically decreased.

Since 2001, I have been touting the benefits of stretching and strengthening exercises to help reduce knee injuries; therefore, help reduce the likelihood of knee pain.

Do your knees a favor if you are currently not incorporating stretches in your daily routine then-START DOING THEM! The simplest stretch you can begin doing TODAY is to sit in bed and have your legs stretched out in front of you. Then bend over from the waist as if you’re going to touch your toes.  Now most of you with knee pain will not be able to touch your toes.  DON’T WORRY ABOUT THAT!! The most important thing is that you feel a pull or stretch down the back of both legs or in your hamstring muscles. Hold this stretch for 10 seconds and repeat it 5 times. If you do this simple stretch every day, without fail you will notice that the knee pain and cramping in your legs will subside.

Tai Chi Helps Relieve Osteoarthritis Knee Pain in Seniors

Recently, a patient with osteoarthritis knee pain asked me if Tai Chi, a form of Chinese martial arts, would help reduce their OA symptoms.   Absolutely!!   The slow rhythmic movements of Tai Chi not only help reduce stiffness and pain in the joint but they also help improve a patient’s mood.

A recent study published online by Arthritis Care and Research, conducted by Tufts University School of Medicine found that Tai Chi, a form of Chinese martial arts, helps seniors improve symptoms of osteoarthritis knee pain.

Researchers from Tufts University School of Medicine divided 40 participants into two groups.  Twenty participants attended Tai Chi classes 2X a week for 12 weeks for 60 minutes.  The other 20 participants attended a 60 minutes class that covered stretches, diet and nutrition and information about the disease.  At the end of the 12 week period the group that practiced Tai Chi noticed a significant decrease in knee pain over the control group.  Researchers also noted an increase in strength, range of motion, and improved overall balance with the group that practiced Tai Chi.

Exercise Induced Leg Cramps or Leg Cramps Associated with Knee Pain?

Exercise enthusiasts, specifically marathon runners often suffer from leg cramps.  An often overlooked remedy for this problem is pickle juice.   NFL athletic trainers use pickle juice, which is a mixture of vinegar, salt and water to help football players stave off muscle cramps after a long training day.   Pickle juice has a ton of sodium, and also offers potassium and magnesium.  All of which have been credited to get rid of exercise induced cramping in the calves.

On the other hand, individuals with knee pain who do not exercise but complain of  having cramps in their calves should not ingest pickle juice since these type of leg cramps are NOT DUE TO EXERCISE.  Instead try stretching your calves. First, put comfortable shoes on, then stand and place the ball of your foot up against the wall and lean into the wall.  Hold this stretch for 10 seconds and repeat this 5 times.  You should feel this stretch in your calf muscle. Of course this stretch won’t get rid of your knee pain, but it will help ease the pain of those cramped calves, which tend to get your attention at night when your trying to sleep.

Muscle Pain Causing Your Knee Pain? Try Eating Ginger

If you’ve never eaten raw ginger you may want to start.  A new study reported in The Journal of Pain, published by the American Pain Society states that taking daily doses of heat-treated or raw ginger helps relieve muscle pain by nearly 25%.  Past research has shown that ginger may have anti-inflammatory and pain relieving or analgesic properties similar to NSAIDs (non-steroidal anti-inflammatories).  So researchers from the University of Georgia and Georgia College and State University examined the effectiveness of ginger on 74 volunteers.  These volunteers were divided into three groups and were either given raw ginger, heated ginger, or placebo.  The volunteers were given ginger for 11 consecutive days after they completed 18 eccentric muscle exercises that induced muscle pain.

The results of the study indicated that the raw ginger lowered the muscle pain intensity by 25% and the heat treated ginger lowered it by 23%. The report did not indicate the amount of ginger given to the volunteers during this study.

Exercise Just as Beneficial as Knee Surgery for the Treatment of Runner’s Knee

A team of researchers from the ORTON Research Institute in Helsinki, Finland conducted a randomized controlled study to determine if arthroscopic knee surgery was more effective than exercise for the treatment of patellofemoral pain syndrome.  Patellofemoral pain syndrome or Runner’s Knee is a common diagnosis given to patients who suffer from knee pain around the front of the knee.  It is most often diagnosed in runners, jumpers, cyclists or in patients who heavily stress their knee joints. Patients usually experience dull, achy pain in the front of the knee and under the knee cap when they climb stairs, squat, or kneel.   Patellofemoral knee pain includes conditions such as chondromalacia patella, anterior knee pain syndrome, and patellofemoral misalignment.

In the study researchers randomly separated 56 individuals into two groups suffering from chronic patellofemoral pain syndrome (PFPS).  The first group underwent an eight week exercise program: while the other group underwent arthroscopic knee surgery, and then an eight week home exercise program.  Nine months later the team assessed the outcomes of both groups by using the Kujala score for knee pain and mobility.  They also determined the cost of care associated with each patient. Interestingly, both groups experienced a decrease in pain and improvement in knee mobility and there was no significant difference in the average Kujala scores between both groups.  Other tests were also conducted but even those didn’t reveal a significant difference in outcomes between the two groups. The only significant difference between the groups was the healthcare costs.  Obviously the group that underwent knee surgery and exercise had higher healthcare costs then the group that only exercised.

Researchers also conducted a two year follow up, and still found that there were no differences in outcomes between the two groups.

The study concluded that exercise was just as effective as arthroscopic knee surgery but less costly for the treatment of patellofemoral pain syndrome, and that arthroscopic knee surgery should not be routinely used to treat knee pain.

Hip Strengthening Exercises May Reduce Patellofemoral Knee Pain in Female Runners

A new research study reported in Science Daily by Tracy Dierks, assistant professor in the Department of Physical Therapy at Indiana University-Purdue University Indiana indicates that female runners who suffer from patellofemoral knee pain (pain behind the knee cap) can reduce or eliminate it significantly with hip strengthening exercises.

Patellofemoral knee pain is a very common running injury that is usually caused by the knee cap not tracking properly, which causes the back of the knee cap to rub up against the thigh bone.  If left untreated the cartilage behind the knee cap will wear down and osteoarthritis will develop.   It is most commonly diagnosed in females due to the angle from the hip to the knee joint being wider in females compared to males.

The study used a pain scale of 0-10 with 3 representing the onset of knee pain and 7 representing strong knee pain while running on a treadmill. Eleven female runners participated in the study and they all started the study with a 7 on the pain scale and ended the study with a 2 or lower.  After performing hip strengthening exercises for 2X a week for 6 weeks they were all able to reduce or eliminate the pain in their knees.

This is a very promising study which suggests that every individual suffering with patellofemoral knee pain should consider trying a hip strengthening exercise program first to see if that helps relieve the pain behind their knee caps.

Most individuals suffering from a meniscus tear, cartilage loss, severe knee arthritis, tendonitis, bursitis, or a baker’s cyst in the knee find it impossible to exercise without experiencing knee pain.  ‘No pain, no gain’ should not be the motto when it comes to exercising if you have been diagnosed with a knee problem.
