If there is a pre-existing meniscus tear or if there is arthritis in the knee joint, and the joint get irritated or injured in some way then the knee may produce excess fluid. A baker’s cyst or popliteal cyst is when synovial fluid (joint fluid) accumulates in the back of the knee.
Signs and Symptoms:
If a baker’s cyst is small in size it may go unnoticed, but as it grows it becomes more symptomatic. A baker’s cyst can get so large that it looks like a ‘tumor’ in the back of the knee, and oftentimes patient’s mistaken it for a blood clot.
Patients with a baker’s cyst will often complain of pain, tightness, and swelling in the back of the knee. The pain may become more severe at night and when the knee is in a bent or in an extended position. Patients may also experience pain in the back of the leg.
Knee Treatment:
Aspirating the fluid from a baker’s cyst is unproductive because the fluid will return. Surgery is never recommended for removing a baker’s cyst since it is a secondary reaction to arthritis or a meniscus tear in the knee joint. Pain behind the knee or pain in back of knee along with swelling and stiffness is often due to a Baker’s Cyst or Popliteal Cyst. Most individuals experiencing pain behind the knee or pain in the back of the knee along with swelling fear they have a blood clot and discover after an ultrasound or MRI is performed that they have developed a Baker’s Cyst.
What is a Baker’s Cyst?
A Baker’s cyst is a pocket of excess synovial fluid. It ONLY develops if a person has a condition in the knee that causes inflammation or swelling. For example, if an individual has a meniscus tear, arthritis or any other knee condition that irritates the tissue around the knee joint then that individual will start producing more synovial fluid. If the knee produces more fluid then the lymphatic system (the system in our body responsible for getting rid of excess fluid, waste, toxins) can drain then the excess fluid starts to accumulate in a sac in the back of the knee. When excess fluid is in the sac it is called a Baker’s Cyst or Popliteal Cyst. A Baker’s Cyst is like a balloon being filled with water. The more fluid in the balloon the larger it gets until it expands to the point where it can no longer take the pressure and the balloon ruptures. If a Baker’s Cyst gets too large it can also rupture. If a baker’s cyst ruptures it is not life threatening but it is extremely painful.
Symptoms of A Baker’s Cyst:
A large Baker’s Cyst can be very painful, and can cause tightness and stiffness into the back of the thigh and calf. A large baker’s cyst not only causes pain in back of knee or pain behind the knee but it can even cause a radiation of pain in the front of the thigh. A Baker’s cyst can cause severe swelling, pressure, and stiffness behind the knee. Most patients are unable to sit in a chair comfortably, kneel, sleep, or stretch their leg out.
Diagnosing and Treating a Baker’s Cyst:
Baker’s cysts are diagnosed with either an ultrasound or an MRI and many times if they are large enough a protrusion can be felt in the back of the knee. Again it’s important to note that Baker’s Cysts or Popliteal Cysts are always SECONDARY to another chronic condition. For example, being diagnosed with a meniscus tear or arthritis in the knee does NOT mean you will develop a Baker’s cyst. However, if you have a Baker’s cyst then you will ALWAYS have either a meniscus tear, arthritis or another condition causing swelling or inflammation in the knee. Baker’s Cysts don’t just appear without cause. There is always a condition in the knee joint that is causing inflammation in order for a baker’s cyst to exist. That is why draining a baker’s cyst is useless because it will reappear. It is also VERY RARE that an orthopedic doctor would attempt to surgically remove a baker’s cyst since it is a symptom of some other condition in the knee, and there are MAJOR arteries and nerves that run too close to the sac to remove it safely.
Another treatment option for treating pain and swelling from a Baker’s Cyst is Cold laser therapy. Cold laser therapy is a treatment that reduces irritation and inflammation to the tissues in and around the knee joint allowing them to heal faster. The following is the true story of Jill V. a 61 year old Plano resident diagnosed with arthritis, a meniscus tear, a Baker’s Cyst, and near bone on bone and the knee treatments she tried before she finally found Dr. Schnee.
“In September of 2009 my pain was focused on the inside of my left knee. From time to time it would vary to the back portion of the knee. Unless one has actually experienced this type of knee pain, it is hard for someone to grasp how intense this can be. The pain was sharp, stabbing, burning and achy. Walking, climbing stairs, sitting, sleeping were all uncomfortable. I had severe swelling in the back of my knee. The pain was constant. I was always groaning every time I took a step. It was crippling me and my well being. It obviously changed my life and not for the better. In December 2009 I needed to seek some options and some answers.
After about 4 months of agonizing knee pain in my left knee I was referred to an orthopedic surgeon by my primary care doctor. First, he took a knee x-ray and an MRI which revealed inflammation, some arthritis, a meniscus tear, a bakers cyst and near bone on bone. He explained what this all meant and I begged him for a cortisone injection. For 4 days I had no pain in my knee. I was brand new, but that didn’t last long. After the four days I was in excruciating pain again. Then he sent me for physical rehabilitation. I did stretches, used heat/ice packs, and lotions. I was still hurting in January of 2010. There was NO RELIEF in sight. So in February he recommended a series of three injections called Euflexxa. These injections don’t tickle. They are painful. They first start out by draining the knee fluid with a syringe and then they follow that with the injection. I believe anyone who has had these injections will tell you they ‘hurt’, this is not an exaggeration. The result of these painful injections? Zero, the Euflexxa did not work on me and the pain never lessened. In March, I was referred to the ‘Brace Guy’. This was just another added expense and the brace was heavy and burdensome. In May 2010 I read about Dr. Schnee’s Cold Laser Knee Pain Solution Program.
She was my last hope otherwise the orthopedic knee doctor told me that we would have to discuss knee surgery.
I completed Dr. Schnee’s Cold Laser Knee Program in July and now I can walk without knee pain, climb stairs with no effort. I can sit and get up from a chair or bed without hesitation on the first step. The swelling in the back of the knee from the Baker’s Cyst is minimal. And I can pretty much do anything I want to do now.

Knee Pain From a Meniscus Tear, Arthritis, And a Baker’s Cyst
On a scale of 1-10 the pain often reached a debilitating 9 or 10 with regularity. I seldom could go an hour without some level of pain. The right side of my left knee was extremely painful when the fabric of my pants touched the skin. I had to wear a knee wrap often to diffuse this. Excruciating pain occurred after riding in or driving a car for more then 10-15 minutes. (My knee) never failed to act up, no matter the seat adjustment. At home I could not sit for a long time without pain occurring. After a while I had to get up and walk the pain off. I tried different types of pain relievers and a cortisone shot. It was only effective for 2-3 weeks then started to wear off.
Now that I’ve completed Dr. Schnee’s Cold Laser Knee Program I can now drive or ride in a car for at least an hour with no debilitating pain. I can now sit for a long time without pain occurring. I am very well pleased with the results. I would rate my improvement at least 70-75% better then when I started. It was well worth the effort and price.
