Chondromalacia Patella – New Study Indicates Knee Osteoarthritis Targets the Patellofemoral Joint

Years ago it was thought that only 10% of the population suffered with osteoarthritis behind the knee cap (or chondromalacia patella).  A recent study released at the World Congress on Osteoarthritis indicates that this percentage is probably a lot higher than once believed.

First Let’s Define Chondromalacia Patella:

The back of the knee cap and the front of thigh bone make up the patellofemoral joint.  When there is osteoarthritis (cartilage loss or cartilage damage) behind the knee cap this condition is called chondromalacia patella. There are different stages of chondromalacia patella depending on the severity.  Grade I is the least severe and Grade IV is the most severe form of osteoarthritis. For example, if a doctor informs you that you have grade II chondromalacia patella it indicates that you have moderate osteoarthritis between the knee cap and thigh bone (the patellofemoral joint). 

Symptoms of Chondromalacia Patella:

Patients with knee osteoarthritis in the patellofemoral joint (or chondromalacia patella) have difficulty getting up from a chair (typically they need to use their arms to push themselves up), they have difficulty squatting, and stairs are typically a problem to climb. Patients will experience pain behind the knee cap anytime the knee is in a bent position and then extended or vice versa.  As the condition becomes more severe it will eventually affect the tibiofemoral joint (the joint that is formed between the thigh bone and shin bone).  This is why most people have osteoarthritis in both the patellofemoral joint (again the joint formed between the knee cap and thigh bone) AND the tibiofemoral joint. 

Most Conventional Knee Treatments for Chondromalacia Have a High Failure Rate…

Most orthopedic surgeons will not focus on the patellofemoral joint because most of the traditional knee treatments available have fairly high failure rates averaging about 30%. Instead surgeons focus on the tibiofemoral joint (the joint that is formed from the thigh bone and shin bone) but a recent study released at the World Congress on Osteoarthritis which involved 970 people found that more people had cartilage damage at the patellofemoral joint then the tibiofemoral joint. 

The National Institutes of Health and the Arthritis Foundation funded the study headed by Dr. Joshua Stefanik Ph.D., a research associate in Boston University’s Clinical Epidemiology, Research and Training Unit; and Dr. David Hunter, a professor of medicine at the University of Sydney.

There were 970 people randomly recruited from the community for the Framingham (Mass.) Osteoarthritis Study.  The mean age was 63.4 and only 22% complained of knee pain.  An MRI was performed and found that 20.4% of the 970 people had cartilage damage in the patellofemoral joint versus only 10.4% had damage in the tibiofemoral joint, and finally 44.2% had damage in both joints.

More People Have Chondromalacia Patella Then Previously Believed…

Since this study clearly indicates that knee osteoarthritis targets cartilage behind the knee cap it is time to change how doctors manage and treat the knee joint.

I was reading the current issue of AARP magazine when I stumbled onto an article that discussed the seven super foods that can help ease pain.  I was really interested in two of the seven foods they were discussing since they had to do with relieving chronic knee pain.  So in this blog I want to give you an overview of the two foods they talked about.(In the past, I’ve written about foods you should avoid or consume if you have chronic knee pain that you check out by clicking onto the link.)

Complaining of Chronic Knee Pain?  Try Taking Ginger Extract

In one of my blogs I specifically wrote about ginger and how it can help relieve knee pain caused from muscle inflammation.  Recently though another study about the benefits of ginger were released by the University of Miami.  The study was conducted with patients who had been diagnosed with chronic knee pain.  This six week study found that almost two-thirds of patients with chronic knee pain experienced less soreness upon standing after taking a ginger extract.  The ginger extract also helped chronic knee pain sufferers walk with less pain after walking 50 feet compared to those taking a placebo.  In addition, those taking the ginger extract reported taking less pain medication.   

It is believed that taking as little as two to three teaspoons of ginger extract daily can block a key enzyme that induces inflammation and promotes knee pain.

Soy Also Helps Relieve Knee Pain

A study from Oklahoma State University stated that patient’s were able to reduce their pain medications by fifty percent after consuming 40 grams of soy protein daily for three months.  For good sources of soy protein, increase your intake of soy milk, edamame, or tofu.  The benefits of soy take at least two to three weeks before they kick in so be patient!

Changing Your Diet Will Make A Difference…

The bottom line is that diet plays a major role in how our bodies react.  You can either have a diet filled with inflammatory promoting foods; for example, red meat, sugar, processed foods OR you can consume more anti-inflammatory foods such as soy and ginger.

Scientists Think Protein in Knee Joint May Ease Arthritis Knee Pain

In healthy knees, synovial fluid is the lubricant that allows the joint to glide.  In an unhealthy arthritic knee there are decreased amounts of synovial fluid.  In the past few years pharmaceutical companies have come out with rooster comb injections (aka gel injections, hyalgan injections, hyaluronic acid injections) to try and replicate the effects of synovial fluid.  However, research indicates that the active protein or molecule in those injections only lasts for a few days in the knee joint before it is absorbed into the bloodstream. 

That is why Saleem Abubacker, a 23 year old PHD student at the University of Calgary in Canada and his colleagues have been researching a different molecule then the one found in current knee injections.  Abubacker and his team are studying the effects of lubricin, a protein found in synovial fluid and on the surface of the knee joint.  They are injecting lubricin in unused cow joints from butcher shops and putting them in a machine that pushes them and twists them like human knee joints.  Preliminary studies suggest that lubricin lasts for longer periods of time in the knee joint (28 days) compared to the current molecule used for injections on the market, and it has shown to slow down the breakdown of cartilage.  They are also finding that the molecule forms building blocks, so as it grows it makes a difference in lubricating the knee.  Abubacker thinks that some patients with arthritis knee pain may have more of the single molecule then the long chain and that by restoring that balance of lubricin they may have less arthritis knee pain.

Obviously, more advanced studies need to be completed with lubricin, especially on human knee joints, but who knows this may be the next type of knee injection in the future.

Cortisone Knee Injections for Knee Pain-What You Need To Know…  

Cortisone knee injections are commonly prescribed by orthopedic doctors as a means of providing temporary relief to knee pain sufferers.  In this blog, I’m going to discuss cortisone injections for knee pain: when to get them, when and why you should avoid them, and why it’s important to consider other knee treatment alternatives. 

Cortisone Knee Injections

Cortisone is considered a steroid that is naturally produced by our bodies (our adrenal glands) in times of stress. However, the cortisone that is injected into the knee joint is not natural it is man made. It is synthetically produced by pharmaceutical companies to help temporarily relieve inflammation and swelling.  For example, Kenalog, and Celestone are all name brands of synthetic cortisone injections.

Cortisone Is NOT A Pain Reliever

A common misconception is that cortisone relieves knee pain.  It is NOT a pain reliever it simply reduces inflammation and swelling in the knee joint. As a result of there being less fluid in the joint, the pressure is alleviated around the sensitive nerve endings and a patient will experience knee pain relief.

When You Should Consider Getting A Cortisone Knee Injection:

Individuals in acute knee pain (those who’ve had a recent sports injury, a car accident, a fall) should consider getting a cortisone injection since it does help temporarily relieve swelling and inflammation in the knee.  The effects of the knee injection can help you get through those first few days or weeks until you can find a REAL SOLUTION to your problem.

Why You Should Avoid Cortisone Knee Injections: 

  • Some patients mistakenly think that they are ‘cured’ after getting an injection, and it gives them a false sense of security since they usually experience pain relief.  Since the relief is just temporary patients should be even more cautious with their knee so as to not injure it further.
  • Long term and repeated use of cortisone will cause bone loss, weakening of tendons, and cartilage loss in the knee joint.  This is why doctors only allow a maximum of three injections per year.
  • Cortisone injections WILL NOT help you if you don’t have swelling or inflammation in the knee joint.
  • Infections are also possible since the needle is injected into the joint.
  • Diabetics have to be monitored when getting cortisone injections since it can spike blood sugar levels.
  • Injections can be painful.
  • Finally, cortisone is not a long term solution to relieving chronic knee pain since it is not repairing or healing the tissue in the joint causing the pain.

Acute Knee Pain VS.  Chronic Knee Pain

In my opinion, if you’ve recently injured your knee and you have swelling, inflammation and pain then a cortisone injection may help you.  However, don’t be fooled into thinking that the effects from the injection will last forever. During that time you should be searching for a REAL solution to your problem.

On the other hand, if you suffer from chronic knee pain for example from bone on bone, arthritis, tendonitis, bursitis, a baker’s cyst, a degenerative meniscus tear, than cortisone should NOT be considered as an option.  After all, cortisone causes cartilage and bone loss, and weakening of the tendons.   The last thing you want to do is to trade one chronic knee problem for another.  I can recall a case where a patient came to see me with a torn patellar tendon.  It tore because it weakened due to the multiple cortisone injections he had over a period of  two years (he had a total of six injections- three injections a year) to relieve tendonitis in his knee.  The injections helped relieve some of his knee pain but what he didn’t realize was that the steroid was damaging and weakening the tendon, and it eventually tore. By the time he came to see me it was too late… he was a surgical case. 

If  he only knew about cold laser therapy for tendonitis knee pain he probably could have avoided the cortisone injections…

Which Knee Treatment is a Better Alternative…

In my clinical experience, (and I’ve treated hundreds of patients with chronic knee pain) cold laser therapy is typically more effective and safer than a cortisone injection. It is a painless, non-invasive treatment tool that allows physicians to eliminate swelling and pain in the knee, but more importantly it is used to help stimulate and accelerate the healing process to repair damaged tissue. 

Cold laser therapy is a better, safer alternative then cortisone because you will experience all of the upsides but none of the side-effects that you would experience with cortisone injections.  In addition, since cold laser therapy works on a cellular level to heal injured, irritated and damaged tissue the results are typically NOT temporary.  On the other hand cortisone does nothing to help tissue repair so even after the injection the problem is still there. Again cortisone only temporarily relieves knee pain by reducing swelling it does nothing to treat the cause of the problem.

A Meniscus Tear…

Pain Behind The Knee |Pain in Back of Knee| Baker’s Cyst

Pain behind the knee or pain in back of knee along with swelling and stiffness is often due to a Baker’s Cyst or Popliteal Cyst.  Most individuals experiencing pain behind the knee or pain in the back of the knee along with swelling fear they have a blood clot and discover after an ultrasound or MRI is performed that they have developed a Baker’s Cyst.

What is a Baker’s Cyst?

A Baker’s cyst is a pocket of excess synovial fluid.   It ONLY develops if a person has a condition in the knee that causes inflammation or swelling.  For example, if an individual has a meniscus tear, arthritis or any other knee condition that irritates the tissue around the knee joint then that individual will start producing more synovial fluid.  If the knee produces more fluid then the lymphatic system (the system in our body responsible for getting rid of excess fluid, waste, toxins) can drain then the excess fluid starts to accumulate in a sac in the back of the knee.  When excess fluid is in the sac it is called a Baker’s Cyst or Popliteal Cyst.  A Baker’s Cyst is like a balloon being filled with water.  The more fluid in the balloon the larger it gets until it expands to the point where it can no longer take the pressure and the balloon ruptures. If a Baker’s Cyst gets too large it can also rupture.  If a baker’s cyst ruptures it is not life threatening but it is extremely painful. 

Symptoms of A Baker’s Cyst:

A large Baker’s Cyst can be very painful, and can cause tightness and stiffness into the back of the thigh and calf.  A large baker’s cyst not only causes pain in back of  knee or pain behind the knee but it can even cause a radiation of pain in the front of the thigh.  A Baker’s cyst can cause severe swelling, pressure, and stiffness behind the knee.  Most patients are unable to sit in a chair comfortably, kneel, sleep, or stretch their leg out. 

Diagnosing and Treating a Baker’s Cyst:

Baker’s cysts are diagnosed with either an ultrasound or an MRI and many times if they are large enough a protrusion can be felt in the back of the knee.  Again it’s important to note that Baker’s Cysts or Popliteal Cysts are always SECONDARY to another chronic condition.  For example, being diagnosed with a meniscus tear or arthritis in the knee does NOT mean you will develop a Baker’s cyst.  However, if you have a Baker’s cyst then you will ALWAYS have either a meniscus tear, arthritis or another condition causing swelling or inflammation in the knee.  Baker’s Cysts don’t just appear without cause.   There is always a condition in the knee joint that is causing inflammation in order for a baker’s cyst to exist.  That is why draining a baker’s cyst is useless because it will reappear.  It is also VERY RARE that an orthopedic doctor would attempt to surgically remove a baker’s cyst since it is a symptom of some other condition in the knee, and there are MAJOR arteries and nerves that run too close to the sac to remove it safely.
