Are you sure that you have No Cartilage in your Knee, and it is Completely Bone on Bone?

I want to clarify the term ‘bone on bone’ which unfortunately is overused and is often incorrectly used when it comes to the knee joint.

Most often patients who have been diagnosed with severe osteoarthritis of the knee are told they are ‘bone on bone’.  Patients automatically think that this means they have no cartilage in their entire knee joint.  When in actuality, if you took an x-ray of their knee, it would probably show a SMALL area or PATCH in their knee joint that had very little cartilage remaining (90% of the time this occurs on the inside or medial aspect of the knee joint).

My point is that the majority of the time there is cartilage in the knee, and if there is a patch of cartilage that has worn down it usually never involves the entire knee joint.  If the entire knee joint was involved, then it would be correctly diagnosed as complete ‘bone on bone’. However, these patients would be wheelchair bound, and not able to straighten or bend their knee.

Since 2001, and after completing over 32,000 cold laser treatments, I’ve only seen a handful of cases that could truly be classified as complete ‘bone on bone’ in the entire knee joint.  ‘Bone on bone’, is a very visual term so orthopedic doctors find it a lot easier to describe a patient’s condition to them as being ‘bone on bone’ rather than explaining that there is only a small patch in the joint that is worn down etc…Unfortunately, these patients leave the surgeons office feeling hopeless and really don’t have a full understanding of their condition so they think they have absolutely no cartilage left in their entire knee and that they need a knee replacement.

Please don’t feel hopeless! If you’ve been told that you are ‘bone on bone’ and that you need a total knee replacement there are other non-invasive options available.  Overall, more research has been conducted with cold laser therapy to help eliminate or reduce osteoarthritis arthritis joint pain than with non-steroidal anti-inflammatory drugs (NSAIDS- the standard prescribed drug) and knee injections. Cold Laser Therapy is safer and frequently more effective than drug therapies, injections and arthroscopic knee surgery.  If you are suffering from osteoarthritis knee pain and are not ready for a total knee replacement then you should consider Dr. Schnee’s Cold Laser Knee Program.

What is the Most Effective, Non-Invasive Treatment Option Available for Knee Pain Relief?

After reviewing the pros and cons of cold laser therapy and comparing them to the most commonly prescribed non-invasive treatment options available for knee pain relief I am sure you will conclude that cold laser therapy is the most effective non-invasive treatment option available.

Let’s begin by discussing the various non-surgical options to treat knee pain:

Cortizone Injections or Steroid Injections :
Besides anti-inflammatories, cortizone injections are one of the most commonly prescribed treatments for knee pain.  Unfortunately, the effects of a cortisone injection are temporary in nature and often times a patient will further injure their knee because the injection masks the pain but does not fix the problem.   Once the cortisone injection wears-off the pain comes back, and sometimes worse than it was prior to getting the injection.  In addition, a doctor will only prescribe three cortisone injections a year because the injections are known to deteriorate bone.  Beware of getting cortisone injections if you are a diabetic it may cause havoc on your sugar levels.

Conclusion: Painful, Short-term and long term side-effects, not safe for long term use, Temporary Relief, Treats symptoms, Diabetics and patients with brittle bones beware.

Synvisc™, Orthovisc™, Hyalgen™, Euflexxa™,  AKA: knee gel injections,  AKA: Rooster comb injections for the knee:
Rooster comb injections are given in a series of 3 or 5 injections over the course of several weeks, and the series can be repeated every 6 months.  If you ask just about any knee pain sufferer that has had these painful injections most will tell you that the injections did NOTHING to help relieve their knee pain.  Unfortunately, if you do notice any knee pain relief it will typically last only 6 months.  Most patients who choose to undergo the series of injections every 6 months over the course of a few years may eventually experience an allergic reaction.  There have been no long-term studies completed so long-term side-effects are unknown.

Conclusion: Painful, Short-term and long-term side-effects, unclear how safe it is for long term use, Temporary Relief, Treats symptoms, Series of injections

Anti-Inflammatory
The results from anti-inflammatories are temporary in nature and patients need to continue taking them on a daily basis in order to feel any results.  In addition, over the long run NSAIDS can cause ulcers and ibuprofen can cause liver problems.

Conclusion: Not painful, Short-term and long term side-effects, not safe for long term use, Temporary relief-needs continuous use to be effective

Cold Laser Therapy (Class IIIB lasers)
Cold Laser Therapy has been around for over 35 years in Europe and Asia, and 15 years in the United States.  There are over 2,500 published studies on the topic of cold laser therapy and (many of them are double blind, placebo controlled studies).  The treatment is FDA cleared, safe, painless, and there are no side-effects with treatments.  Unfortunately, cold laser therapy is not a quick fix especially when treating chronic knee pain.  Even though patients may notice improvement after just a few treatments most patients undergo a series of treatments over a 5-10 week period depending on the severity of their knee problem.  Once the treatments are completed most patients will notice that the results are not temporary in nature. It is not uncommon that as a result of cold laser treatments symptoms such as knee pain, swelling, immobility may be completely eliminated or substantially reduced.  Patients suffering with severe and chronic knee pain may need periodic maintenance treatments. Also make sure you choose a doctor that specializes in using cold lasers to treat knee pain otherwise you make not get the results you deserve.

Conclusion: Safe, painless, no side-effects, typically knee pain relief is NOT temporary in nature, takes a series of treatments and then possible periodic follow ups are needed, to get the best results pick a doctor that treats only knees and specializes in cold laser therapy.

Cold Laser Therapy is Found to Reverse the Inflammatory Process of Arthritis, and Increase Biosynthesis of Cartilage

A new study analyzed the effects of cold laser therapy on chronic arthritis induced in  rats using hydroxyapetite (gives rise to rheumatism and is found in over 20% of joints with osteoarthritis)and calcium pyrophosphate (this provokes chondrocalcinosis)crystals.  Scientists separated 56 rats equally into seven groups:

Group A was a control group.
Group B was injected with HP (hydroxyapetite) for two weeks.
Group C was injected with HP and was treated with laser.
Group D was injected with CaPP  (calcium pyrophosphate) for two weeks.
Group E was injected with CaPP and treated with laser.
Group F was injected with a physiologic solution.
Group G was only treated with laser and NOT injected at all.

Scientists collected and examined tissue samples from the  knee joint  using light microscopy.   The results of this study found that Group C and Group E, the rats that were induced with arthritis and were treated with laser for two weeks, had an increase in the biosynthesis of cartilage, the inflammatory process was reversed, and that the arthritic process was slowed down.   Scientists also noticed that structural alterations in the knee joint that developed due to arthritis significantly diminished after the cold laser treatments.

The other two groups, Group G (which had laser alone but did not have any injections), and Group F, (the group that was injected with physiologic solution) produced no histological tissue changes.  This finding is important because it indicates that cold laser therapy only affects and treats damaged tissue and doesn’t affect a healthy joint. It also indicates that the injections had nothing to do with the positive results of the study.1

This study  proves , yet again, that Cold laser therapy is a viable treatment for those seeking a non-invasive treatment option to reduce the adverse effects of arthritis, and don’t want the adverse effects typically seen with ‘conventional medical treatments’.

Cold Laser Therapy Helped Intense Knee Pain When Surgery and Injections Failed

Before coming in for cold laser therapy with Dr. Schnee the pain in my left knee was intense!  It felt like someone took a chisel, put it on the inner side of my left leg and banged away.  Each step I took hurt.  I wasn’t able to stand and put any pressure on my left knee.  Going up and down the stairs was nearly impossible.
