Laser Education 101: The History of Lasers
LASERs were first theorized by Albert Einstein in 1917. In 1960, Dr. Endre Mester, a Hungarian scientist, invented laser technology. Prior to being FDA Cleared in the U.S. cold lasers were being used in Europe, Japan and China to treat acute and chronic conditions. In addition, there were at least 2,000 studies already published proving the efficacy of cold laser therapy. Years later, and after significant developments in the technology, cold lasers were finally cleared in the U.S. in 2001.
Currently, there are multiple types of cold lasers that are FDA cleared for the treatment of a variety of health conditions including arthritis pain. In addition, cold laser therapy is used in many hospitals, rehabilitation, and clinics in the U.S. and around the world. In Europe, China, and Japan, it is not unusual for patients to undergo cold laser treatments to treat chronic and acute conditions in order to reduce pain, swelling and restore function. In the United States almost every professional sports team and professional athlete uses cold laser therapy to treat injuries. Cold lasers allow them to speed up the healing process, decrease pain, increase range of motion, and eliminate swelling.
Laser therapy has also proven to be a versatile and high-tech solution in the health care field. By simply changing the wavelength, frequency and power of a cold laser it can be used for various health conditions. For example, it is becoming more common for orthopedic doctors, pain management specialists, cosmetic surgeons, dentists, dermatologists, and podiatrists to use cold lasers as part of their arsenal of tools in assisting patients with their treatment goals.

Doctors Are Still Prescribing Useless Arthroscopic Knee Surgeries and Gel Injections for Arthritic Knees
Why is it that arthroscopic knee surgery and gel injections are recommended by surgeons even though there is overwhelming proof that these procedures DON’T WORK?
It is common knowledge in the medical field that arthroscopic knee surgery for arthritic knees is a useless procedure that clean outs and removes the cartilage/meniscus in the knee joint. The surgery actually contributes and exacerbates the very condition it’s supposed to treat….ARTHRITIS.
I believe that if patients diagnosed with arthritic knees fully understood the consequences of arthroscopic knee surgery they would not consent to undergo this procedure. Instead they would demand safer alternatives- that made sense.
Injections are also another option orthopedic surgeons offer patients suffering from arthritis knee pain. But the results from SYNVISC and other gel injections are questionable and the results are not much better than arthroscopic knee surgery. (Lately, the injections are losing popularity because patients are discovering that they are useless, and multiple studies have long proven them to be ineffective. Just ask patients who’ve undergone a series of them.)
Why is it that cold laser therapy which has been around for over 30 years, used all over the world, has thousands of double blind research studies proving its effectiveness, and does not have any side effects is hardly ever offered by an orthopedic surgeon as an option to arthritic knee pain?
Just follow the money…
There are surgeons that don’t refer patients out for cold laser therapy because they never come back to them for surgery.
It’s a shame.
My belief is doctors should work as a team to provide patients the VERY best care. I am not against knee surgery. I am against needless knee surgery. There are times a patient has no other choice, but to undergo knee surgery. If a patient needs surgery and understands the consequences of knee surgery they should have it done. However, if a patient does not want or need knee surgery then ALL of their options should be reviewed including cold laser therapy.
Often patients have the perception that if their doctor doesn’t offer cold laser therapy or doesn’t know about it then it must be ‘quackery’. Cold laser therapy is based on decades of proven science and is not ‘quackery’. The results of cold laser treatments are far more impressive then injections or arthroscopic knee surgery. Honestly, as a patient you should be more skeptical about arthroscopic surgery then cold laser therapy. Just read the studies. Furthermore, there are thousands of published, double blind research studies proving cold laser therapy’s effectiveness.
For years I’ve had patients come to see me for arthritic knee pain and I have not had one person tell me that the arthroscopic knee surgery or the rooster comb injections provided any significant pain relief. Yet everyday in my practice I have patients with arthritic knees undergo our Cold Laser Knee Program and get considerable pain relief. Most notice enough knee pain relief and an increase in strength in their knee joint that they are able to avoid knee replacement surgery.

Do The Series of ‘Gel Like’ Injections Really Work?
I’m referring to Hyaluronic acid injections aka rooster comb, gel injections, viscosupplements, SYNVISC®, Hyalgen®, Euflexxa®, Supartz®, Orthovisc®, etc.
In my opinion the answer to this question is NO, NOT REALLY. Ultimately though you have to decide for yourself… Let me explain…
I have personally treated and talked to many patients who have had the series of three to five injections listed above (3-5 injections are usually administered over the course of a few weeks. Recently, SYNVISC® came out with one injection). Most patients noticed little to no knee pain relief after the injections. Some noticed that their knee pain and swelling got worse after the injections and they developed what’s known as a ‘hot knee’. Others developed an allergic reaction to the shot and were unable to complete the series. Of the few that responded favorably to the injection they noticed temporary pain relief (which in all fairness the injections do claim that the relief is only temporary and usually lasts for up to 6 months).
There are multiple studies indicating that Hyaluronic injections are as effective as placebo injections,[1] [2] [3]and that the effect of anti-inflammatory drugs is 48 times greater than the effect of Hyaluronic acid injections (or rooster comb injections).[4]
One study indicated that the immediate pain relief patients experienced was not due to the injection itself, but from draining the fluid in the knee, a step doctors take prior to administering the injection to prep the knee.
Most patients mistakenly think that the injections add ‘cushioning’ or cartilage to their knee. Unfortunately, this is not the case! IF the injections are successful the theory behind them is that they help lubricate the knee joint. The injections do NOT stimulate cartilage or increase the amount of cartilage in the knee.
Based on my personal experience and the multiple studies I have read (not the ones funded by the pharmaceutical companies, but ones written by a third party) about the injections. I believe that there isn’t any real evidence that proves their effectiveness in the treatment of osteoarthritis knee pain.
On the other hand, I do believe that an individual should leave knee surgery as their very last option, so it’s certainly worth giving it a ‘shot’.

Do Hyaluronic Acid Injections for Arthritis Knee Pain Really Work?
No! Not according to a study published in the Journal of the American Medical Association.
Researchers found that hyaluronic acid injections (rooster comb injections) given in the knee joint (for instance Hyalgan, Synvisc, Artzal, Suplasyn, BioHy, or Orthovisc) at best, had a small effect in the treatment of knee OA (osteoarthritis) compared with a placebo injection … Approximately 80% of the treatment effect of intra-articular hyaluronic acid was accounted for by the placebo effect of an intra-articular injection.
The study concluded that there was no real evidence to show that hyaluronic acid injections are more effective than placebo injections.
Even though the US Food and Drug Administration approved hyaluronic acid injections for the treatment of arthritis knee pain its effect is still controversial. Unfortunately, the reason that hyaluronic acid injections are controversial is that most of the research studies published about the injections have been funded by the developers of the drug and therefore are considered bias.
The study mentioned above was performed by a third party, and to our knowledge was not funded by a drug company.

Knee Pain Was So Severe That Patient Could Not Put Weight On Her Knee, Had To Walk With A Limp, And Could Not Stand For Any Length Of Time

Are Synvisc Shots (Rooster Comb) Better then Cortisone Shots for Osteoarthritis Knee Pain?
The Journal of Bone and Joint Surgery (American) published a study comparing the results of cortisone and Synvisc injections in patients diagnosed with osteoarthritis knee pain. One hundred patients with knee osteoarthritis were randomized to receive an injection in the knee joint of either Hylan G-F 20 (Synvisc) or a corticosteroid, and they were followed for six months. The patients treated with Hylan G-F 20 (Synvisc) received one course of three weekly injections. The patients treated with the corticosteroid received one injection at the time of enrollment in the study, and they could request one more injection any time during the study. An independent, blinded evaluator assessed the patients with the Osteoarthritis Western Ontario and McMaster UniversityIndex (WOMAC), a modification of the Knee Society rating system, and the visual analog pain scale.
Researchers concluded that after a six month follow up that there were no detected differences in knee pain or function between patients treated with Hylan G-F 20 injections (Synvisc) then those treated with a cortisone shot. Interestingly, women demonstrated significantly less response to treatment than men did for both treatments on all three outcome scales.

Vitamin D Deficiency Contributes To Cartilage Loss and Knee Pain
A simple blood test to determine Vitamin D levels may give the insight needed to determine whether a total knee replacement is in an individual’s future. Cartilage loss or the term ‘bone on bone’ is the hallmark of osteoarthritis. However, “achieving vitamin D sufficiency in osteoarthritis patients could significantly delay total knee replacement,” said Dr.Ding, at the Menzies Research Institute in Australia.
Researchers found that patients diagnosed with osteoarthritis that have sufficient levels of vitamin D have approximately 1.5 percent less loss of knee cartilage per year than patients with vitamin D deficiency.
The investigators measured levels of vitamin D in blood samples and knee cartilage volume on X-rays from 880 men and women who were 51 to 79 years old. Researchers then took similar measurements again almost 3 years later among 353 of the study participants.
Overall, 58 percent of these subjects that had Vitamin D deficiency showed changes in knee cartilage indicating worsening osteoarthritis between the first and second measurements, and half reported knee pain.
The study concluded that vitamin D deficiency contributed to lower knee cartilage volume and patients were more likely to experience knee pain.
Besides contributing to cartilage loss or osteoarthritis of the knee, Vitamin D deficiency is known to cause many other health problems. It is worth getting your blood levels tested for Vitamin D, and fixing any deficiency in order to delay the progression of osteoarthritis and the need for total knee replacements.
